动态心电图检测短阵性室性心动过速的临床分析  被引量:4

摘  要:目的对动态心电图检测短阵性室性心动过速(SVT)的临床意义进行研究。方法纳入2015年8月至2016年7月30例SVT患者,设为观察组,同期30例偶见室性早搏患者设为对照组。按照SVT起源心室不同,将观察组分为左心室组(14例)和右心室组(16例)。将对照组和观察纽的震荡斜率、震荡初始、窦性心率震荡分级情况及异常概率进行比较,同时比较左心室组和右心室组患者的平均心室率、QT离散度、QRS时限以及QRS电轴。结果观察组和对照组的震荡斜率、震荡初始、窦性心率震荡分级(0级和2级)、异常概率存在较大差异(P〈0.05),左心室组和右心室纽患者的QRS电轴差异较大(P〈0.05),但平均心室率、QT离散度、QRS时限比较接近(P〉0.05)。结论动态心电图中的窦性心率震荡可预测心律失常的发生情况,QRS电轴可作为室速心室起源的判断标准。Objective To study the clinical significance of dynamic electrocardiogram test in the supraventricular tachycardia. Methods From August 2015 to July 2016, 30 cases of paroxysmal ventricular tachycardia were selected as observation group, 30 cases of occasional ventricular premature beat were set as control group. In accordance with the paroxysmal ventricular tachycardia ventricular, the observation group were divided into left ventricular group ( 14 cases ) and right ventricular group ( 16 cases ) . The turbulence, turbulence onset, heart rate turbulence classification and anomaly probability in the control group and the observation group were compared at the same time, compared to the rate, QT dispersion, QRS duration and QRS axis in average ventricular group and right ventricular group. Results The observation group and control group of turbulence slope, shock initial and grade of sinus heart rate turbulence ( 0 and 2 ) , abnormal probability had significant difference ( P〈0.05 ) , left ventricular and right ventricular group in patients with a greater difference between QRS electrical axis ( P〈0.05 ) , but the average degree of ventricular rate, QT dispersion, closed to QRS duration (P〉0.05) . Conclusion Sinus heart rate turbulence in dynamic electrocardiogram can predict the occurrence of arrhythmia, QRS electrical axis can be used as the origin of ventricular tachycardia ventricular judgment standard.
