摘　　要：目的系统评价水痘减毒活疫苗保护效果,为优化水痘减毒活疫苗免疫程序提供依据。方法计算机检索Medline、EMbase、Cochrane Library、万方数据库、CNKI数据库、维普数据库、CBM,收集水痘减毒活疫苗保护效果的独立队列研究、病例对照研究以及横断面研究,并追溯所获文献的参考文献,检索时限截至2016年3月。按纳入和排除标准筛选文献、提取资料和评价质量后,采用R 3.1.0进行统计分析。结果最终纳入35项研究,共11 204例水痘病例和409 985例对照。Meta分析结果显示,水痘减毒活疫苗总保护率为75%(95%CI:68%~80%),OR=0.25(95%CI:0.20~0.32)。对幼儿和小学生的保护率分别为90%(95%CI:81%~95%)和67%(95%CI:52%~78%)。暴发疫情中水痘疫苗的保护率为59%(95%CI:47%~68%)。结论水痘减毒活疫苗具有一定的保护效果,但随着时间的推移,保护效果降低。Objective To systematically evaluate the vaccine effectiveness（VE）of varicella attenuated live vaccine among children and provide basis for optional immune program. Methods Databases as Medline,Embase,Cochrane Library,Wanfang Data,CNKI,CQVIP,and CBM were searched for reports including cohort,case-control and cross-sectional studies on VE of varicella attenuated live vaccine from January 1998 to March 2016.Meanwhile,the references of relevant studies were retrieved.Inclusion and exclusion criteria were used for the screening of literatures.The statistical analysis was performed using R 3.1.0 software. Results A total of 35 studies,including 11 204 cases and 409 985 controls were finally analyzed.Meta analysis revealed that the overall VE of varicella attenuated live vaccine was 75%（95% CI：68%-80%）The VE of varicella attenuated live vaccine was 90%（95%CI：81%-95%）among preschool children and 67%（95%CI：52%-78%）among elementary school students. Conclusions Varicella attenuated live vaccine is moderately effective in preventing varicella,but the VE reduces over time.
