摘 要:目的利用磁共振T2mapping成像技术定量分析内侧半月板损伤程度与内侧胫股关节软骨T2值的相关性。方法选取43例正常和单纯内侧半月板后角损伤患者,行常规MRI和T2mapping扫描。半月板损伤分0-Ⅲ级,根据损伤程度分为对照组GO组,实验组G1、G2、G3。根据WORMS评分将内侧胫股关节软骨分为股骨内侧中区、后区,胫骨内侧前区、中区和后区5个亚区,经工作站T2Map软件后处理,分别测量各组各亚区软骨浅、深层T2值。结果GO、G1、G2、G3组各组各亚区间软骨浅、深层T2值差异均有统计学差异(P〈0.05),呈由浅层向深层减低趋势;G2、G3组胫骨内侧后区软骨浅、深层软骨T2平均值均高于GO组(P〈0.05),但GO、G1组间各亚区软骨浅、深层T2平均值间差异无统计学意义(P=0.06、0.73),且G3组软骨深层T2值高于G2组(P=0.01〈0.05)。G3组股骨内侧中区及胫骨内侧中区软骨浅、深层软骨T2平均值均高于GO组(P〈0.05)。结论内侧半月板损伤与内侧胫股关节软骨损伤存在相关性,T2mapping成像能够发现半月板功能减退引起的软骨早期退变。Objective To evaluate the relationship between the severity of medial meniscus and medial tibiofemoral cartilage' s T2 value. Methods After strict screening, 43 patients with normal or posterior horn injury of the medial meniscus were included, and routine MRI and T2 mapping of knees were performed. According to the injury degree, the medial meniscus injuries were scored on a scale of 0--Ⅲ,where control group G0;experiment group G1,G2, G3. According to the WORMS score, the medial tibiofemoral joints were divided into five subregions : central medial femoral region, posterior medial femoral region, anterior medial tibial region, central medial tibial region, posterior medial tibial region. After the post-processing of the workstation, superficial and deep layers' T2 value of each subregion were measured in each group respectively. Results Spatial differences in cartilage T2 values of each group' s subregion were statistically different(P 〈0.05), which shows a trend of decrease from superficial to deep layer ; The T2 value of posterior medial tibial region cartilage' s superficial and deep layer in G2,G3 groups were all higher than those of G0(P〈0.05). However, the differences of T2 values between the GO and Glgroups had no statistical significance(P = 0.06,0.73). What' s more, the deep layer' s T2 value of G3 posterior medial tibial region showed significantly higher than that of G2(P=-0.01〈0.05). The T2 value of cartilage' s superficial and deep layer in the central medial femoral region and Central medial femoral region of G3 were all higher than those of G0(P〈0.05). Conclusion High grade demage of the medial meniscus showed significant association with higher T2 values. T2 maoping may be a useful tool in detecting early cartilage degeneration when meniscus function is lost.
