成人脐疝巴德改良Kugel补片腹膜前修补术20例临床分析

摘　　要：目的探讨巴德改良Kugel补片腹膜前修补方法治疗成人脐疝的临床效果。方法回顾性分析2010年3月至2016年3月武汉科技大学附属孝感医院收治的成人脐疝病人20例的临床资料,其中男性8例,女性12例,年龄28~81岁,病史2~15年,合并肝硬化3例,合并慢性阻塞性肺疾病5例,合并心房纤颤7例,腹腔镜术后并发脐疝5例。全部病人行腹部CT检查,明确肚脐处缺损大小。脐环直径2.0~4.5 cm。全部采取改良Kugel补片行腹膜前修补。结果手术时间为(40±10)min,术中出血(20±5)ml,术后即可进食、下床活动,术后无需止痛,术后2~3 d出院。术后无明显脐部异物感。电话随访5个月至5年无复发及慢性疼痛感。结论改良Kugel补片腹膜前修补方法是治疗成人脐疝的理想方法,值得临床推广使用。Objective To investigate the clinical effect of adult umbilical hernia repair using MK patch through preperitoneal way.Methods A retrospective analysis was performed on the clinical data of 20 adult patients with umbilical hernia admitted from March 2010 to March 2016 in our hospital using the MK patch through preperitoneal way.Results The operation time was 40±10 min,the blood loss was 20±5 mL.The patients could get out of bed and had food intake,and there was no need to relief pain after operation.The patients were discharged in 2-3 days after operation.No obvious foreign body sensation was found in the umbilical region.No recurrence and chronic pain were found during a follow-up period of 5 months-5 years.Conclusions The MK preperitoneal repair method is an ideal method for the treatment of adult umbilical hernia,and it is worthy of clinical use.
