益气养阴活血方联合雷公藤多苷片治疗糖尿病肾病Ⅳ期临床研究

摘　　要：目的:观察在常规西医疗法基础上加用益气养阴活血方联合雷公藤多苷片治疗糖尿病肾病Ⅳ期患者的临床效果。方法:纳入40例糖尿病肾病Ⅳ期气阴两虚兼血瘀证患者,随机分为对照组、联合组各20例。对照组给予常规西医疗法治疗,联合组在对照组治疗基础上加用益气养阴活血方、雷公藤多苷片治疗。2组均治疗12周。治疗前后检测24h尿蛋白定量(24h UAE)、血浆白蛋白(Alb)、血肌酐(SCr)、谷丙转氨酶(ALT)、血白细胞计数(WBC),计算预估肾小球滤过率(eGFR),观察记录不良反应发生情况。结果:治疗后,2组24h UAE均较治疗前下降(P<0.01,P<0.05),联合组24h UAE低于对照组(P<0.05)。对照组eGFR与治疗前比较,差异无统计学意义(P>0.05);联合组eGFR较治疗前升高(P<0.05);联合组eGFR高于对照组(P<0.05)。联合组Alb高于对照组(P<0.05);联合组SCr和症状积分均低于对照组(P<0.05,P<0.01);2组ALT、WBC比较,差异均无统计学意义(P>0.05)。结论:在常规西医疗法基础上加用益气养阴活血方联合雷公藤多苷片对糖尿病肾病Ⅳ期患者具有明显减少尿蛋白、延缓肾功能恶化等作用,无明显不良反应。Objective： To observe the clinical effect of Yiqi Yangyin Huoxue prescription combined with Tripterygium Wilfordii polyglycoside tablets in treating patients with DN of Ⅳstage, based on the routine western medicine therapy. Methods： Divided40 cases of patients with DN of Ⅳ stage with Qi-Yin deficiency syndrome into the control group and the combination group randomly, 20 cases in each group. The control group was treated with routine western medicine therapy, while the combination group was treated with Yiqi Yangyin Huoxue prescription combined with Tripterygium Wilfordii polyglycoside tablets. Both groups received treatment for 12 weeks. Detected 24 hour urinary albumin excretion（24h UAE）, plasma albumin（Alb）, serum creatinine（SCr）, alanine transaminase（ALT）, White blood cell count（WBC）before and after treatment, calculated estimated glomerular filtration rate（eGFR） and observed incidence of adverse reaction. Results： After treatment, 24 h UAE of the two groups was decreased when compared with that before treatment（P〈0.01, P〈0.05）, 24 h UAE of the combination group was lower than that of the control group（P〈0.05）. After treatment, compared eGFR of the control group with that before treatment, there had no significant difference（P〈0.05）; eGFR of the combination group was higher than that before treatment（P〈0.05）; eGFR of the combintion group was higher than that of the control group（P〈0.05）. The levels of Alb of the combination group were higher than those of the control group（P〈0.05）; SCr and symptom scores of the combination group were both lower than those of the control group（P〈0.05, P〈0.01）. Compared levels of ALT and WBC of the two groups,there had no significant difference（P〈0.05）. Conclusion： Yiqi Yangyin Huoxue prescription combined with Tripterygium Wilfordii polyglycoside tablets can reduce urine protein and delay renal function deterioration effectively in treating patients with DN of Ⅳstage, which has no obvious
