结核性支气管食管瘘一例并文献复习

摘　　要：作者总结分析了山东省胸科医院收治的1例结核性支气管食管瘘患者的临床表现,以及辅助检查、诊治过程、随访等病历资料,并结合文献进行回顾性分析.该患者因＂间断发热4个月,胸骨后疼痛2个月,进食呛咳4 d＂入院,经胸部CT、气管镜及胃镜检查确诊为结核性支气管食管瘘,经抗结核药物治疗、食管支架置入并行气管镜下注药、冻融等治疗,患者支气管食管瘘瘘口愈合. 通过文献复习认为结核性支气管食管瘘临床罕见,经胸部CT、气管镜及胃镜等检查可明确诊断,进行积极的抗结核药物治疗及内镜下食管支架置入、局部注药、冻融等联合治疗有助于瘘口愈合.The clinical,auxiliary examinational and pathological data of a case with tuberculous bronchoesophageal fistula in Shandong Chest Hospital was collected,and the literature were analyzed retrospectively.The patients was admitted to hospital because of intermittent fever for 4 months,retrosternal pain for 2 months and eating cough for 4 days, It was diagnosed by chest CT, bronchoscopy and gastroscope examination.After treatment of anti-tuberculous therapy,esophageal stented local injection and freeze thawing treatmen,the bronchial esophageal fistula was cured.Through the literature review, tuberculous bronchoesophageal fistula was found is rare, and could be diagnosed by the chest CT scan, bronchoscope and gastroscope test.Anti-tuberculous therapy and endoscopic esophageal stenting, local injection and freeze thawing treatment contributed to the healing of the fistula.
