针刺结合五苓汤加减治疗强直性脊柱炎的临床研究

摘　　要：目的:观察针刺结合五苓汤加减治疗强直性脊柱炎的疗效及安全性。方法:将108例强直性脊柱炎患者按随机数字表法分为西药组、中药组和针药组各36例。西药组给予柳氮磺吡啶片,中药组给予五苓汤加减,针药组给予针刺结合五苓汤加减,疗程均为60 d,观察各组患者治疗前后总和指数(BAS-G)、活动性指数(BASDAI)、放射学指数(BASRI)、活动度衡量指数(BASMI)和中医辨证强直性脊柱炎(中医证候)的评分,检测治疗前后血沉(ESR)、C-反应蛋白(CRP)、甲状旁腺激素(PHI)和补体3(C_3)的变化,比较各组临床疗效及不良反应发生率。结果:治疗后针药组总有效率71.4%,优于西药组的58.1%和中药组的52.8%,针药组各评分均较西药组和中药组改善更为显著,针药组和中药组不良反应发生率低于西药组。结论:针刺结合五苓汤加减可显著提高强直性脊柱炎患者BAS-G、BASDAI、BASRI、BASMI和中医证候评分,改善ESR、CRP、PHI和C_3水平且安全有效。Objectives： To explore the therapeutic effect in the treatment of ankylosing spondylitis with acupuncture combined with modified Wulingtang. Methods： A total 108 cases of ankylosing spondylitis were randomly divided into the chemical medicine group,the Traditional Chinese medicine（ TCM） group and the acupuncture combination with the TCM group,36 cases in each group. Modified Wulingtang was treated in TCM group; sulfasalazine tablets was applied to the chemical medicine group; the acupuncture combined with modified Wulingtang was adopted in the acupuncture combination with TCM group. 60 d for a course. The bath ankylosing Spondylitis Patient Global Score（ BAS-G）,the bath ankylosing spondylitis disease activity index（ BASDAI）,the bath ankylosing spondylitis irradiation index（ BASRI）,the bath ankylosing spondylitis metroloty index（ BASMI） and ankylosing spondylitis by TCM syndrome differentiation（ TCM symptom） were applyied to estimate the efficacy of the 3 group before and after treatment. Erythrocyte sedimentation rate（ ESR）,C-reactive protein（ CRP）,parathyroid hormone（ PHI） and complement3（ C_3） were detected. Results： The efficiency of the acupuncture combination with TCM group was52. 8%. After treatment,the acupuncture combination with TCM group all of score was superior the others; The detection of ESR,CRP,PHI and C_3 in the acupuncture combination with TCM group is better than the other two groups. Conclusion： Treating Ankylosing spondylitis with the acupuncture combination with modified Wulingtang is remarkable.
