摘 要:目的 观察糖尿病性视网膜病变(DR)患者广泛视网膜光凝术(panretinal photocoagulation,PRP)后黄斑区脉络膜厚度的变化.方法 对重度非增生性糖尿病性视网膜病变(NPDR)32例(50眼)行PRP术,以25例(36眼)正常眼作为对照组.应用海德堡频域光学相干断层扫描,观察PRP术前及术后1周、1、3个月黄斑中心凹脉络膜厚度(SFCT),以及距黄斑中心凹750 μm的上方、下方、鼻侧、颞侧共5个位点脉络膜厚度.结果 PRP术前NPDR患眼脉络膜厚度比正常对照组薄.PRP术后1周时,患眼脉络膜厚度较术前显著增加,术后1个月与术后1周相比,脉络膜厚度明显变薄,术后3个月与术后1个月相比,患眼脉络膜厚度更薄.结论 通过频域光学相干断层扫描可以直观地了解黄斑区脉络膜厚度,NPDR会导致黄斑区脉络膜变薄,PRP术短期内能够显著增加脉络膜厚度,但随治疗后时间延长,其厚度会逐渐降低.Objective To observe the the changes of macular choroidal thickness after panretinal photocoagulation(PRP)in patients with diabetic retinopathy(DR).Methods Fifty eyes of 32 patients with non-proliferative diabetic retinopathy(NPDR)underwent PRP,and 36 eyes of 25 normal subjects were selected as control group.All cases received examination by Heidelberg SD-OCT before and 1 week,1,3 months after PRP.Subfoveal choroidal thickness(SFCT),and superior,inferior,nasal,temporal choroidal thickness 750μm around macular central fovea were measured.Results The macular choroidal thickness of the eyes with NPDR before PRP was thinner than that of normal subjects.One week after PRP,the choroidal thickness of NPDR group was significantly thicker than that before PRP.The choroidal thickness got thinner from 1 week to 3 months after PRP.Conclusion The thickness of macular choroid can be seen intuitively by the SD-OCT,and it tends to thin caused by NPDR.The choroidal thickness can be significantly increased after PRP in the short term,but choroid thickness will gradually get thinner with time after treatment.
