锁骨远端骨折钩板内固定术后再骨折原因分析及处理

摘　　要：目的分析锁骨远端骨折钢板内固定术后再骨折原因及防治措施。方法应用锁骨钩板治疗锁骨远端骨折发生再骨折11例,全部病人均得到随访,随访时间4个月~1.5年,平均(9.2±2.5)个月。结果 186例病人出现再骨折11例,住院期间发生8例,术后1个月2例,3个月内1例,发生外伤4例,轻微外伤7例,均发生于NeerⅡ型骨折术后。结论锁骨远端骨折钩板内固定术后再骨折发生的原因包括解剖因素、手术操作不当和术后功能锻炼不合理。Objective To analyse the re-fracture causes and prevention measures of distal clavicle fracture fixed by plate. Methods 146 cases were used clavicular hook plate for distal clavicle fracture. 11 of them had refractured. All 11 cases were followed up for four months to 1. 5 years. the average followed time was（ 9. 2 ± 2. 5） months. Results There were 11 patients re-fractured in 186 cases. In this11 cases,8 cases occurred during hospitalization,1 case occurred one month later after surgery,1 case occurred within three months after surgery. And in this 11 cases,four cases are trauma,seven cases are minor trauma,they all occurred after Neer Ⅱ fracture. Conclusion The reasons of re-fracture of distal clavicle fracture using hook plate fixed are： anatomy,improper operation in surgery and extral exercises after surgery.
