摘 要:目的:通过使用我院改造的F4输尿管导管,提高输尿管镜钬激光碎石术处理复杂性输尿管中下段结石的有效性和安全性。方法:回顾性分析我院自2015年1月~2016年6月收治的55例复杂性输尿管中下段结石患者的临床资料:55例患者均伴有轻中度不等的肾积水,其中有39例患者伴输尿管狭窄,13例患者伴有息肉或肉芽组织包裹,3例患者行体外冲击波碎石(ESWL)治疗失败后伴石街形成。55例患者在接受输尿管镜钬激光碎石治疗过程中,全部应用我院自行改造设计的F4输尿管导管。结果:55例患者手术时间在18-65min之间,其中有53例患者单次手术碎石成功,单次手术成功率达96.4%(53/55),2例患者在钬激光碎石中结石上移至肾盏,后期再行ESWL治疗;55例患者术中均无输尿管黏膜剥脱、输尿管穿孔以及假道形成等并发症。结论:采用我院自行改造设计的F4号输尿管导管应用于输尿管镜钬激光碎石术中,可针对复杂性输尿管中下段结石发挥良好的作用,可有效缩短手术时间并显著降低输尿管术中、术后相关并发症,该技术值得应用和推广。Objective: By using the F4 ureteral catheter modified by our hospital, To improve the efficacy and safety of ureteroscopic holmium: YAG laser lithotripsy in the treatment of complex middle and lower ureteral calculi. Methods: The clinical data of 55 patients with complex middle and lower ureteral calculi treated in our hospital from January 2015 to June 2016 were retrospectively analyzed: Fifty - five patients accompanied by mild to moderate hydronephrosis, Thirty - nine patients with ureteral stricture, Thirteen patients with polyps or granulation tissue wrapped, Three patients underwent extracorporeal shock wave lithotripsy (ESWL) after treatment failure with stone. Fifty -five patients received ureteroscopic holmium: YAG laser lithotripsy with the F4 ureteral catheter of our hospital transformation. Results: The operation time of Fifty -five patients was between 18 -65min, There were 53 patients with single operation, the success rate of single operation was up to 96. 4% (53/55), Two patients in the holmium laser lithotripsy stones showed no ureteral mucosa avulsion, ureteral up to the calyceal and then treated with ESWL; Fifty- five patients perforation and other complications via formation. Conclusion: Using the F4 ureteral catheter of our hospital transformation used in ureteroscopic holmium : YAG laser lithotripsy, It can play a important role in complex middle and lower ureteral calculi and, It can effectively shorten the operation time and significantly reduce the intraoperative and postoperative complications, which is worthy of application and promotion.
