摘 要:目的探讨晚期早产儿生后早期振幅整合脑电图(aEEG)变化特点,并分析影响aEEG的临床因素。方法分析65例晚期早产儿aEEG变化。探讨胎龄、宫内营养、窒息和临床严重疾病等因素对aEEG影响。采用χ~2检验、非参数检验及Logistic回归分析进行统计学分析。结果 (1)胎龄越长,晚期早产儿aEEG评分越高(χ~2=13.6,P=0.01);(2)窒息严重程度越重,aEEG异常发生率越高(χ~2=15.3,P=0.02);(3)合并临床严重疾病组aEEG异常发生率高(χ~2=11.6,P=0.01);(4)Logistic回归分析提示,合并临床严重疾病是aEEG结果异常的独立危险因素(OR=1.2,95%CI:1.1~10.7,P=0.04);(5)除临床严重疾病因素外,窒息也是aEEG结果异常独立危险因子(OR=1.5,95%CI:1.5~15.1,P=0.01);(6)aEEG成熟度评分与NBNA评分正相关(r=0.77,P=0.02)。结论随着胎龄增长,晚期早产儿aEEG逐渐成熟,但窒息和临床严重疾病等因素是影响aEEG的独立危险因素,临床上应关注合并高危因素的晚期早产儿脑功能变化。Objective To investigate the characteristics and risk factors of late preterm infants in amplitude integration characteristic of EEG(aEEG). Methods We analyzed 65 cases in late preterm infants who received aEEG exam after birth in our hospital. We analyzed the effect of gestational age,intrauterine nutrition,perinatal asphyxia,and the clinical situation on aEEG. Chi-square test,nonparametric test,and Logistic regression analysis were used for statistical analysis. Results(1)With gestational age growth,the aEEG scores of late preterm became higher(χ^2=13.6,P=0.01);(2)The aEEG abnormal rate was higher in group severe asphyxia group(χ^2=15.3,P=0.02).(3)The aEEG abnormal rate was higher in group with serious illness(χ^2=11.6,P=0.01).(4)Logistic regression analysis indicated that severe disease was an independent risk factor in late preterm with abnormal aEEG(OR=1.2,95% CI:1.1~10.7,P=0.04);(5)Excluding systemic serious illness factors,asphyxia was also a risk factor(OR=1.5,95% CI:1.5~15.1,P=0.01);(6)After adjusting the gestational age to full-term age,the aEEG score and NBNA score coincidence rate was 52.0% . The aEEG maturity score positively correlated with NBNA score(r=0.7,P=0.02). Conclusions With gestational age growth,late preterm aEEG matures gradually. The asphyxia and serious illness factors are independent risk factors that influence the aEEG. We should pay attention to the brain function changes in the late preterms with risk factors.
