军事飞行员下肢深静脉血栓形成1例并文献复习

摘　　要：目的探讨下肢深静脉血栓形成对军事飞行员飞行的影响及航空医学鉴定原则。方法分析1例军事飞行员下肢深静脉血栓形成的诊疗过程、预后及医学鉴定,从中总结经验、教训,并复习相关文献。结果该例飞行员因"右小腿不适"初步诊断为右小腿软组织感染,最终明确诊断右下肢深静脉血栓形成,排除各种遗传性及获得性病因,经过积极抗凝处理,相关辅助检查指标明显好转,症状明显减轻。结论早发现、早治疗对军事飞行员下肢深静脉血栓形成具有重要意义,鉴于对飞行职业影响较大,建议根据具体情况个别评定。Objective To explore the influence of lower extremity deep venous thrombosis（LEDVT） on the performance of a military pilot and evaluation principles of aeromedicine. Methods By analyzing the process of diagnosis and treatment, prognosis and medical evaluation of one military pilot with LEDVT, we summarized the experience and lessons, and reviewed the related literature. Results The ＂right calf discomfort＂ of the pilot was diagnosed as soft tissue infection of the right leg at first, but the final diagnosis was LEDVT after ruling out the possibility of various genetic and acquired diseases. The related auxiliary examination indexes improved and the symptoms were alleviated significantly through the anticoagulation therapy. Conclusion Early discovery and treatment of LEDVT is of great significance for military pilots. We recommend personalized assessment according to specific conditions.
