腔内激光治疗术与静脉剥脱术治疗大隐静脉曲张疗效比较

摘　　要：目的探讨腔内激光治疗术和静脉剥脱术2种方法治疗大隐静脉曲张的临床疗效。方法选取2015年10月—2016年10月于宝鸡市中医医院手术治疗的单侧大隐静脉曲张患者128例纳入课题研究,根据随机数表法分为激光治疗组(64例)和剥脱手术组(64例),分别行大隐静脉高位结扎联合腔内激光治疗术和大隐静脉高位结扎剥脱术。比较2组患者平均手术时间、平均术中出血量、术后并发症的发生率、术后恢复时间及术后复发率的差异。结果腔内激光治疗组的平均手术时间、术中出血量和术后恢复时间均明显优于静脉剥脱组(P<0.001);而2组术后并发症的发生率和再通率差异无统计学意义(P>0.05);2组术后3个月的CEAP分级较术前明显改善(P<0.05)。结论大隐静脉腔内激光治疗联合高位结扎术是治疗大隐静脉曲张的安全、有效的微创方法。Objective To compare the curative effects of treating varicosis of the great saphenous vein with endovenous laser and stripping of the great saphenous vein. Methods The clinical data of 128 patients with unilateral great saphenous varicose veins who had been treated in our department between October 2014 and October 2015 were divided into two groups using the random number table method. Sixty-four of these patients underwent high ligation of the great saphenous vein combined with endovenous laser treatment and the rest were treated with high saphenous vein ligation and stripping. Results The endovenous laser treatment group was obviously superior to the vein stripping group （P〈0.001） in the duration of surgery, intra-operative blood loss and postoperative recovery time, but the incidence of postoperative complications and recurrence was not different between the two groups （P〉0.05）. CEAP classification of the two groups three months after operation was improved significantly （P〈0.05）. Conclusion The combination of high ligation of the great saphenous vein with endovenous laser treatment is a safe and effective micro-invasive treatment for varicosis of the great saphenous vein.
