HPV分型检测在337例子宫颈细胞学异常者的应用分析

摘　　要：目的分析HPV核酸分型检测在子宫颈细胞学(TCT)异常患者中的临床应用。方法对2015年1月~2016年11月在我院行TCT检查出结果异常的337例患者进行HPV核酸分型,并阴道镜下活检,对比HR-HPV阳性与病理阳性者之间的关系,分析HPV核酸检测在发现宫颈上皮内瘤变及宫颈癌(SCC)中的应用价值。结果 337例TCT异常者中,病理阴性者245例(72.7%),病理为CINⅠ50例(14.8%)、CINⅡ28例(8.4%)、CINⅢ13例(3.8%)、SCC 1例(0.3%);病理阳性92例中HR-HPV检出阳性率(87例,94.5%),明显高于病理阴性(14例,13.5%),差异有统计学意义(P<0.05);其检出CIN及SCC的敏感度为94.5%,特异性94.2%,阳性预测值88.5%,阴性预测值为97.1%,准确率93.3%。结论 HPV分型检测对于年龄≥35岁的TCT异常者进行分流及治疗有重要临床意义。Objective To explore the application of HPV genotyping to the detection of cervical intraepi- thelial neoplasia （CIN） and cervical cancer. Methods From January, 2015 to November,2016, the Thinprep cytology test （TCT） found that 337 cases had abnormal results. HPV genotyping and pathological biopsy under colposcopy were conducted. The HPV-positive results and the pathological-positive results were compared, and the HPV genotyping clinical val to intraepithelial neoplasia （ analyzed. Results Among the CIN） the 3 ue of the application of detection of cervical and cervical cancer was 37 abnormal cervical cases, 245 cases（72.7%）were found pathologically negative, 50 cases （14.8%） were of CIN ：I , 28 cases （8. 4%） were of CIN II ,13 cases （3.8%） were of CIN III and one case （0.3%） was of cervical cancer. Of the 92 pathologically positive cases, the positive rate of HR-HPV detection was 94.5%, higher than that（13. 5 %）of pathologically negative cases（ P 〈0.05）. The sensitivity of HPV genotyping in detecting CIN and cervical cancer was 94.5%, the specificity was 94.2%, the positive predictive value was 88.5%, the negative predictive value was 97.1% and the accuracy rate was 93.3%. Conclusion HPV genotyping detection can effectively help to distribute and cure abnormal cervical patients above 35.
