基于Meta分析的丹红注射液治疗急性脑梗死临床评价研究

摘　　要：目的:采用Meta分析方法评价丹红注射液辅助西医常规治疗急性脑梗死的有效性、安全性。方法:计算机检索Cochrane Library,Pub Med、Embase、Sino Med、中国知网、维普期刊数据库和万方数据库有关丹红注射液治疗急性脑梗死的随机对照临床试验文献,检索时限为各数据库建库至2016年8月。提取资料通过Stata 13.1软件进行Meta分析。结果:共纳入57个随机对照试验,累计5 474例患者。Meta分析结果显示,丹红注射液辅助西医常规可提高临床总有效率(RR=1.62,95%CI:1.46~1.79,P<0.01),提高患者日常生活活动能力(MD=9.61,95%CI:4.80~14.42,P<0.01),改善神经功能缺损(MD=-4.34,95%CI:-5.13^-3.54,P<0.01),改善血液流变学指标。有17篇文献明确无不良反应,3篇研究报道了共12例不良反应,其他文献均未对安全性做出说明。结论:丹红注射液辅助西医常规治疗急性脑梗死具有较好的疗效和安全性,值得临床推广。Objective ：To evaluate systematically the clinical effectiveness and safety of Danhong injection （DH） on acute cerebral infarction （ACI）. Methods： Randomized controlled trims （RCTs） regarding ACI treated by DH and rou- tine western medicine were searched in the China National Knowledge Infrastructure Database, China Scienceand Technology Journal Database, Wanfang Database, Chinese Biomedical Literature Database, PubMed and Cochrane Library up to August, 2016. Two reviewers independently retrieved RCTs and extracted information. A Meta-analysis was conducted with Stata 13.1 software. Results：57 studies involving 5 474 participants were included. Meta-analysis, the results of DH group was significantly superior to the routine western medicine group in recovery rate （RR = 1.62, 95% CI： 1.46-1.79, P 〈 0.01 ） , the activities of daily living （MD = 9.61, 95% CI： 4.80-14.42, P 〈 0.01 ）, the neurologic impairment （ MD = -4.34, 95% CI： -5.13- -3.54, P 〈0.01 ] . Additionally, DH also can improve the level of hemodynamic indices. 17 articles cleared that there was no ADRs in the studies, 3 studies reported 12 ADRs. There were 9 cases in the experimental group, and 3 in control group. Conclusion： Comparing with routine western medicine, the combine use of DH was more ef- fective, and it worthwhile for spreading in clinical practice.
