摘 要:目的:比较高雄烯二酮血症型多囊卵巢综合征(PCOS)患者与非高雄烯二酮血症型PCOS患者在代谢指标以及性激素等临床特征的差异并予以分析。方法:按照血清雄烯二酮水平的差异将入选的98名PCOS患者分为两组:高雄烯二酮血症组(HA,A>3.3ng/mL)、非高雄烯二酮血症型(NA,A≤3.3 ng/mL),并分析两组患者BMI、年龄、腰臀比(WHR)、性激素、血脂、胰岛素抵抗指数稳态模型评估法(HOMAIR)等生化指标上的组间差异性。结果:98名患者中,HA患者73例,NA患者25例。NA组在WHR、睾酮、游离睾酮指数(FAI)以及HOMA-IR水平上明显低于HA组(P<0.05),雄烯二酮与WHR、FAI、LH/FSH比的相关性显著(P<0.05),与睾酮相关性极显著(P<0.001)。结论:HA与NA患者在性激素以及代谢水平上存在差异,提示雄烯二酮检测对于PCOS的诊治具有实际的临床意义。Objectives: To explore the difference in metabolism indexes and sex hormone between polycys- tic ovarian syndrome (PCOS) patients with hyperandrostenedionemia (HA) and those with normal androstenedione (NA). Methods: The data of 98 PCOS patients were collected from Zigong Maternal and Child Health Hospital and grouped into HA group and NA group according to the androstenedione level. The statistical difference between the two groups in BMI, age, LH/FSH, serum liquid, HOMA - IR and other important indicators was analyzed by one - way ANOVA. Results: 73 (74.48%) PCOS patients were divided into HA group, and 25 (25.52%) patients were divided into group NA group. Comparative analysis showed NA group and HA group had a significant difference in the levels of testosterone, androstenedione, WHR, IRI and FAI ( P 〈 0. 05 ). The level of WHR and androsten- dione in HA group significant higher than that of NA group, but no significant difference was found in other indica- tors. The level of testosterone and androstenedione in NA group were significantly lower than that in HA group. Re- lationship analysis showed androstenedione was significant associated with FAI ( R = 0. 298, P = 0. 003 ), WHR ( R =0. 229, P = 0. 004), LH/FSH ratio ( R = 0. 218, P = 0. 0031 ) and testosterone ( R = 0. 373, P 〈 0. 001 ). Conclusion: The significant difference in metabolism indexes and sex hormone between HA and NA PCOS patients suggests that the detect of androstenedione is of significance for the diagnosis and treatment of PCOS.
