摘 要:目的:通过分析某中青年体检人群的体成分的分布情况,探索体内胰岛素水平的影响因素,为胰岛素水平的估计提供简便快捷的方法。方法:(1)选取某医院健康体检人群共205人,测空腹状态下的体成分、血糖(FBG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDLC)、胰岛素等。胰岛素取自然对数,记为Lnins。(2)比较不同年龄组,不同性别上述指标的差异,以及中心性肥胖率的差异。并以Lnins为因变量,建立多元线性回归方程。结果:(1)各年龄组身高、体重、腰围、身体质量指数(BMI)、LDL-C及TG男性均高于女性,差异有统计学意义(P<0.05);随着年龄增加,平均体重、腰围、体脂肪率、BMI、FBG逐渐升高,女性平均Lnins有下降趋势。(2)在20~岁和30~岁年龄组,男性中心性肥胖发生率高于女性,差异有统计学意义,P<0.05;随着年龄的上升,男性和女性中心性肥胖发生率逐渐升高,女性增速更快。(3)Lnins的影响因素为BMI、年龄,回归方程Y=0.036+0.084×BMI-0.007×年龄,R2=0.262,P<0.01。结论:同年龄组比,男性发生肥胖,血糖升高、血脂紊乱等代谢性疾病的风险更高;随着年龄增长,各项指标均朝向不利于健康的方向发展,且女性更为显著。Lnins与BMI呈正相关,与年龄呈负相关。Objective: To study the characteristic of body composition,and analyze the relationship between body composition and Lnins,to provide an easy and convenient method to estimate insulin in blood. Methods:( 1) A total of 205 subjects taken part in a routine health check were investigated in the hospital. After overnight fasting,they were checked for body composition,serum glucose( FGB),triglyceride( TG),total cholesterol( TC),highdensity lipoprotein cholesterol( HDL-C),low-density lipoprotein cholesterol( LDL-C),insulin. Because of insulin data's seriously skewed distribution,their natural logarithm,which named Lnlep and Lnins were chose in analysis;( 2) The differences of the above-mentioned indexes and the rate of central obesity were compared between different age groups,different genders,and the multiple linear regression equation was set up with Lnins as the dependent variable. Results:( 1) In all age groups,height,weight,waist circumstance,BMI,LDL-C and TG were significantly higher in male than in their female counterparts,( all P 0. 05); but HDL-C was lower. With the increasing of age,both genders showed decreased mean height,waist circumstance,BMI,and FBG and overweight rate.( 2) In the 20 ~ and 30 ~ age groups,overweight rates for men's were higher then women's.( 3) The influential factors of Lnins in the population were BMI and age. Y = 0. 036 + 0. 084 × BMI-0. 007 × age,R^2= 0. 262,P 0. 01.Conclusions: In all age groups,the male subjects have higher risk to have obesity and metabolic disorders. Along with the increase of age,all of the selected parameters tend to be abnormal in both genders,especially the females;Lnins has significantly positive correlation with BMI,and has negative correlation with age.
