煤工尘肺壹期晋期贰期常见影响因素分析

摘　　要：目的探讨煤工尘肺壹期晋期贰期的常见影响因素,以期掌握影响尘肺晋期的规律。方法选择某矿业集团煤工尘肺壹期晋期贰期的病例2 419例,采用回顾性调查研究的方法对该矿业集团1961—2015年诊断的煤工尘肺病例资料进行收集,分析不同诊断年代、不同工种、合并结核对煤工尘肺壹期晋期贰期的影响。结果(1)不同诊断年代1组晋期率最高,2组接近于1组,5组高于3、4组。(2)以纯掘工种煤工尘肺壹期晋期贰期时间最短(8.69±4.23)年,混合工种晋期年限最长为(10.33±4.27)年;提示掘进工种可能影响煤工尘肺的晋期时间。(3)合并肺结核的尘肺晋期与未晋期煤工尘肺比较差异有统计学意义(P<0.05)。结论尘肺壹期晋期贰期可能与其诊断年代、掘进工种和是否合并结核有关,掘进工种晋期年限最短。Objective To investigate the common influencing factors on pneumoconiosis with phase I promoting phase Ⅱ among coal workers.Methods A total of 2 419 cases of coal workers with pneumoconiosis With phase I promoting phase Ⅱ （CWP） were collected from a mining group company.Through medical records review, data of coal workers diagnosed with pneumoconiosis between the years of 1961 and 2015 were collected and analyzed to determine the influence of diagnosed year, workpost and tuberculosis on pneumoconiosis phase I promoting phase II. Results ①In accordance with diagnosed year, the promotion time of group 1 was the shortest, with group 2 close to group 1 .The promotion time of group 5 was longer than group 3 and 4.②The phase I promoting phase II time among coal workers of pure digging type was the shortest（8.69 ± 4.23）years while in mixed sectors the promotion time was the longest （10.33 ± 4.27）years.Digging work may affect the promotion time of pneumoconiosis. ③ The difference of promotion time among coal workers with tuberculosis from those with no pneumoconiosis were statistically significant different（P〈0.05）.Conclusion The results showed that the time of pneumoconiosis phase I promoting phase Ⅱ may be related to the diagnosed year and tuberculosis and digging work.The time of pneumoconiosis phase I promoting phase Ⅱ among coal workers involved with digging work was the shortest.
