重组人碱性成纤维细胞生长因子雾化吸入用于全身麻醉下颈椎前路术后咽喉痛30例疗效观察

摘　　要：目的观察重组人碱性成纤维细胞生长因子(rh-bFGF)雾化吸入对全身麻醉(简称全麻)下经颈椎前路术后咽喉痛的疗效。方法将全麻下经颈椎前路术后拔除气管导管的60例患者随机分为治疗组和对照组,各30例。治疗组给予rh-bFGF 35 000 U+0.9%氯化钠注射液20 m L,压缩雾化吸入,每天1次,每次20 min,连续3 d;对照组给予0.9%氯化钠注射液5 m L+地塞米松5 mg+庆大霉素8万U+糜蛋白酶4 000 U,压缩雾化吸入,每天2次,每次20 min,连续3 d。结果治疗组术后12,24,48,72 h咽痛发生率及咽痛(吞咽时)疼痛视觉模拟(VAS)评分均明显低于对照组(P<0.05)。结论 rh-bFGF雾化吸入能明显促进呼吸道黏膜修复,减少出血及支气管狭窄等并发症,从而降低经颈椎前路气管插管全麻术后咽痛发生率及咽痛程度,值得推广。Objective To observe the clinical efficacy of aerosol inhalation of recombinant human basic fibroblast growth factor（ rh-bFGF） for treating sore throat after anterior cervical operation under general anesthesia. Methods Totally 60 patients underwent anterior cervical operation under general anesthesia and removed tracheal catheters were randomly divided into the treatment group and the control group. The treatment group was given rh-bFGF 35 000 U ＋ 0. 9% Sodium Chloride Injection 20 m L,compression inhalation,1 time a day,20 min for each time,the drugs were continuously used for 3 days. The control group was given 0. 9% Sodium Chloride Injection5 m L ＋ dexamethasone 5 mg ＋ gentamicin 80 000 U ＋ chymotrypsin 4 000 U,compression inhalation,twice a day,20 min for each time,the drugs were continuously used for 3 days. Results 12,24,48,72 h after operation,the incidence of sore throat and VAS score of sore throat（ swallowing） of the treatment group were significantly lower than those of the control group（ P 〈 0. 05）.Conclusion Aerosol inhalation of rh-bFGF can obviously promote the repair of respiratory tract mucous membrane,reduce bleeding and bronchial stenosis and other complications,in order to reduce the incidence of sore throat and the degree of pain after anterior cervical operation under general anesthesia,it is worthy of promotion.
