复方鳖甲软肝片联合恩替卡韦治疗HBeAg阴性乙型肝炎肝纤维化疗效及对血清炎性因子的影响

摘　　要：目的观察复方鳖甲软肝片联合恩替卡韦治疗乙型肝炎e抗原(HBe Ag)阴性乙型肝炎肝纤维化的疗效及对血清干扰素-γ(IFN-γ)、白细胞介素-6(IL6)的影响。方法选取医院收治的HBe Ag阴性乙型肝炎肝纤维化患者102例,按照随机数字表法分为观察组和对照组,各51例。对照组给予恩替卡韦治疗,观察组在对照组基础上联合复方鳖甲软肝片治疗,观察两组总胆红素(TBIL)、丙氨酸氨基转移酶(ALT)、白蛋白(ALB)、天门冬酸氨基转移酶(AST)、血清Ⅲ型前胶原(PCⅢ)、透明质酸(HA)、层粘连蛋白(LN)、Ⅳ型胶原(Ⅳ-C)以及IFN-γ,IL-6水平。结果治疗后两组TBIL,ALT,AST,PCⅢ,HA,LN,Ⅳ-C,IFN-γ和IL-6水平显著低于治疗前(P<0.05),且观察组显著低于对照组(P<0.05),两组ALB水平显著高于治疗前(P<0.05),且观察组显著高于对照组(P<0.05)。结论复方鳖甲软肝片联合恩替卡韦能有效改善HBe Ag阴性乙型肝炎肝纤维化患者肝功能和IFN-γ,IL-6水平,并降低肝纤维化程度,值得临床推广。Objective To observe the clinical efficacy of Compound Biejiaruangan Tablets combined with entecavir for treating HBe Ag negative hepatitis B complicated with hepatic fibrosis and its effect on serum IFN-γ and IL-6. Methods Totally 102 patients with HBe Ag negative hepatitis B complicated with hepatic fibrosis were randomly divided into the observation group and the control group,51 cases in each group. The control group was treated with entecavir,while the observation group was treated with Compound Biejiaruangan Tablets combined with entecavir. The levels of total bilirubin（ TBIL）,alanine aminotransferase（ ALT）,albumin（ ALB）,aspartate aminotransferase（ AST）,procollagen Ⅲ（ PC Ⅲ）,hyaluronic acid（ HA）,laminin（ LN）,Ⅳ collagen（ Ⅳ-C）,IFN-γ and IL-6 were observed. Results After treatment,the levels of TBIL,ALT,AST,PC Ⅲ,HA,LN,Ⅳ-C,IFN-γ and IL-6 in the two groups were significantly lower than before treatment,and the observation group was significantly lower than the control group（ P 〈 0. 05）. The levels of ALB in the two groups were significantly higher than before treatment,and the observation group was significantly higher than the control group（ P 〈 0. 05）. Conclusion Compound Biejiaruangan Tablets combined with entecavir for treating HBe Ag negative hepatitis B complicated with hepatic fibrosis can effectively improve the liver function,the levels of IFN-γ and IL-6 of the patients,and reduce the degree of hepatic fibrosis,which is worthy of clinical promotion.
