体外循环手术中桡动脉穿刺处弹力绷带法与普通绷带法固定包扎比较研究

摘　　要：目的:比较体外循环手术中桡动脉穿刺处使用弹力绷带和普通绷带固定包扎法对术中及术后并发症及舒适度的影响。方法:将2010年10月-2013年10月422例在我院手术室行全麻下体外循环手术患者随机分为观察组(212例)和对照组(210例)。观察组的患者桡动脉穿刺处(手术中桡动脉穿刺处行监测患者动脉血压)采用弹力绷带法固定包扎,对照组的患者采用普通绷带固定包扎。并观察固定效果、术后并发症及舒适度情况。结果:两组不同固定包扎方法的固定效果、术后并发症及舒适度的观察比较,观察组的动脉针滑脱、皮下淤血、皮下血肿、伤口渗血、舒适度分别为0.47%、2.35%、0.94%、1.42%、94.34%,对照组分别为3.33%、10.47%、4.29%、5.71%、3.33%、77.14%。结论:体外循环手术桡动脉穿刺处采用弹力绷带固定包扎,术中及术后并发症较普通绷带固定包扎明显减少,舒适度明显增加,可在临床上推广使用。Objective： To compare effects of elastic bandage and ordinary bandage to fix and dress the radial artery puncture point in extracorporeal circulation operation on intraoperative and postoperative complications as well as postoperative comforts. Methods： A total of 422 patients having undergone the general anesthesia extracorporeal circulation operation in our hospital operating room were randomly divided into observation group（ 212 cases） and control group（ 210 cases）. The radial artery puncture point of the observation group（ monitoring the arterial blood pressure in the radial artery puncture operation） was fixed and dressing with elastic bandage method,while that of the control group was dealt with the ordinary bandage. The fixed effect,postoperative complications and comfort conditions were observed. Results： In the observation group,the arterial needle slippage rate,subcutaneous congestion rate,subcutaneous hematoma rate,bleeding wound rate,and comfort degree were 0.47%,2.35%,0.94%,1.42%,and 94.34%,separately,while in the control group,those were 3.33%,10.47%,4.29%,5.71%,3.33%,and 77.14%,separately. Conclusions： For the fixing and dressing of the radial artery puncture point in the extracorporeal circulation operation,the elastic bandage could significantly reduce the intraoperative and postoperative complications and improve the comfort degree than the ordinary bandage,and could be used in the routine clinical applications.
