内镜下治疗结肠息肉在日间病房与住院病房中的回顾性病例对照研究

摘　　要：目的对比结肠息肉患者在日间病房与住院病房中的治疗效果、患者满意度差别。方法收集我科2013年10月至2014年9月日间病房结肠息肉(病灶≤1.0 cm)患者100例与住院病房同类患者100例,两组患者均行肠镜下治疗。采集两组患者临床基本信息,统计住院时间、住院费用、服务满意率、手术并发症发生率、院内感染发生率、局部复发率、完整切除率,并运用t检验及χ2检验对比。结果两组患者临床基本信息差异无统计学意义,日间病房组较普通病房组的住院时间、住院费用、院内感染率低,服务满意率高,差异有统计学意义(P<0.01),两组患者病灶完整切除率、局部复发率、手术并发症发生率、院内感染率、术后局部复发率差异无统计学意义(P>0.05)。结论日间手术病房模式较住院病房模式治疗效果无差别,但可降低院内感染率、治疗费用,缩短治疗周期,提高患者治疗获益程度。Objective To compare the treatment outcome and satisfaction in the patients with colorectal polyps undergoing endoscopic surgery in one-day ward and hospitalized ward. Methods Twohundred patients with colorectal polyps（ ≤ 1. 0 cm） treated with endoscopic surgery in our institute from October 2013 to September 2014,were enrolled in this study. There were 100 patients in one-day ward and100 patients in hospitalized ward. The cost length of hospital stay,rate of service satisfaction,surgical complications,nosocomial infection,local recurrence,complete resection were compared between the 2 groups of patients. Student＇s t test and Chi-square test were performed for comparison between the groups. Results There were no obvious differences in the baseline data between the 2 groups. The ambulatory surgery group had significantly shorter length of hospital stay,lower hospital costs and nosocomial infection,and higher satisfaction rate than the control group（ P 〈0. 01）. No differences were found in total resection rate,local recurrence,or postoperative complications（ P 〈0. 05）. Conclusion There is no significant difference in treatment outcome between the one-day ward and hospitalized ward in the patients with colorectal polyps undergoing endoscopic surgery. However,one-day ward has lower rate of nosocomial infection,less cost,shorter hospital stay,and thus more benefit for the patients.
