摘　　要：目的探究2014-2015年开封市水痘患者经济负担情况及影响因素,为制定水痘防控策略提供参考依据。方法选取2014年1月至2015年12月期间《中国疾病预防控制信息系统》报告的开封市水痘患者,按照随机分层随机抽样的方式,共抽取598人为本调查的调查对象;通过查阅医疗机构的收费系统、患者或家属提供的费用单据或者直接面访患者、电话调查为补充获取患者在门诊及住院的医疗费用的详细信息,主要包括患者发病就诊情况、直接医疗费用及间接医疗费用。结果患者的性别构成数据比较差异无统计学意义;城市水痘患病人群高于农村,城市患者的直接医疗费用及间接医疗费用高于农村;<15岁的水痘患者人群数量及总花费均低于≥15岁的水痘患者;医院等级方面比较结果显示,市级及以上花费高于县级乡镇级及以下的医院,接收的患者多于于县级乡镇级及以下的医院;首次就诊的患者较多。总费用、直接医疗费用市级及以上医院高于县级及以下医院,男性高于女性,年龄≥15费用高于<15岁的患者。直接非医疗费用城市高于农村,间接费用年龄≥15费用高于<15岁的儿童,不同就诊次数的患者费用差异无统计学意义。结论水痘造成患者的经济负担加重,地区、医院等级、年龄、就诊次数为影响水痘患者经济负担的主要影响因素。Objective To investigate the economic burden and influencing factors of varicella patients in Kaifeng City in 2014-2015, and to provide reference for the prevention and control strategy. Methods Patients with varicella Kaifeng period from January 2014 to December 2015 ＂control information system＂ report Chinese disease prevention, according to a stratified random sampling method, a total of 598 people were surveyed in this investigation; through access to medical institutions, patients or families of toll system charges documents or directly interview and telephone surveys for more patients the information patients in the outpatient and inpatient medical expenses for the supplement, including the incidence of patients, direct medical costs and indirect medical costs. Results Patient gender data showed no significant difference; the city of varicella patient groups was higher than that of rural areas, direct medical costs and indirect medical costs of patients in city than in rural areas; the number of patients with varicefla and total costs were lower than those of 15 years old 〈15 years old patients with varicella; hospital level comparison shows that spending is higher than the municipal level and above county level the township level and below the hospital received patients than in the township level and below the hospital; first visit patients more. The total cost, the direct medical cost of municipal level and above hospitals above and below the county level hospital, male is higher than female, aged 15 years old were higher than that of 〈15 cost. The direct medical cost of the city than in the countryside, the indirect costs cost is higher than the age of more than 15 children 〈15 years of age, there was no significant difference in patients with different treatment times of the cost. Conclusion The economic burden of patients with varicella was aggravated, and the main factors influencing the economic burden of patients were the area, hospital grade, age and number of visits.
