血府逐瘀汤联合胺碘酮对急性心肌梗死患者红细胞免疫功能和心功能指标的影响  

Effect of Xuefu Zhuyu Decoction and Amiodarone on Indexes in Red Blood Cell Immune Function and Cardiac Function of Patients with Acute Myocardial Infarction

摘　　要：目的:观察血府逐瘀汤联合胺碘酮治疗急性心肌梗死的临床疗效及对红细胞免疫功能和心功能指标的影响。方法:将300例急性心肌梗死患者随机分为对照组和试验组,每组150例。对照组给予胺碘酮静脉注射治疗,试验组在对照组基础上加服血府逐瘀汤治疗,方药组成:桃仁12 g,当归9 g,生地黄9 g,桔梗4.5 g,甘草6 g,红花9 g,川芎4.5 g,赤芍6 g,枳壳6 g,柴胡3 g。300例患者在治疗前后均检测红细胞免疫功能、左室射血分数(left ventricular ejection fraction,LVEF)、血清胱抑素C(serum cystatin C,CysC)和肌钙蛋白Ⅰ水平(cardiac troponinⅠ,cTnⅠ);以及比较两组患者的临床疗效,并记录治疗过程中出现的不良反应及其应对措施。结果:对照组有效率为84.00%,观察组有效率为98.00%,两组比较,差异有统计学意义(χ~2=21.642,P<0.001);两组患者治疗后红细胞免疫功能评分均有下降,两组比较,差异有统计学意义(P<0.001);两组患者治疗后LVEF升高,两组比较,差异有统计学意义(P<0.001)、两组患者治疗后Cys C下降,两组比较,差异有统计学意义(P<0.001);两组患者治疗后心肌肌钙蛋白Ⅰ(cardiac troponinⅠ,cTnⅠ)水平下降,两组比较,差异有统计学意义(P<0.001)。结论:血府逐瘀汤联合胺碘酮可以提高急性心肌梗死患者的红细胞免疫功能、心功能指标,使患者气血运行通畅,具有降低血液黏度,延缓心肌缺血的功能,临床疗效显著。Objective： To explore the effect of Xuefu Zhuyu Decoction combined with Amiodarone on indexes in red blood cell immune function and cardiac function of patients with acute myocardial infarction. Methods： 300 patients with acute myocardial infarction were randomly divided into control group and experimental group,with 150 cases in each group. Intravenous injection of Amiodarone was given to both groups. The experimental group,in addition to that of the control group,was also treated with Xuefu Zhuyu Decoction. The prescription composition： Peach 12 g,Angelica 9 g,Rehmannia glutinosa 9 g,Jiegeng 4. 5 g,Licorice 6 g,Safflower 9 g,Chuanxiong 4. 5 g,Red peony root 6 g,Bitter orange 6 g,Chaihu 3 g. The red blood cell immune function,LVEF,CysC,cTnI and the treatment effect of the two groups of patients before and after treatment were compared. The adverse reactions in the course of treatment and the corresponding measures were recorded. Results： Compared with that of the control group,the curative rate of the experimental group was significantly higher（ χ2= 21. 642,P〈 0. 001）. After treatment,the red blood cell immune function scores of the two groups were decreased,but that of the experimental group dropped decreased more（ t = 47. 380,P 〈0. 001; t = 20. 681,P〈 0. 001） and LVEF increased（ t = 8. 660,P〈 0. 001; t = 3. 910,P〈 0. 001）,Cys C decreased（ t = 6. 628,P〈 0. 001; t = 2. 646,P = 0. 009） and decreased cTnI levels（ t = 33. 818,P〈 0. 001; t = 22. 546,P〈 0. 001）. Conclusion： Xuefu Zhuyu Decoction combined with Amiodarone can significantly improve the red blood cell immune function and cardiac function of patients with acute myocardial infarction,and help patients＇ blood circulation more smoothly by reducing blood viscosity and delaying myocardial ischemia,which has significant clinical efficacy.
