靳三针联合鼠神经生长因子治疗小儿痉挛型脑瘫临床研究

摘　　要：目的:观察靳三针联合鼠神经生长因子治疗小儿痉挛型脑瘫的临床疗效。方法:选取132例痉挛型脑瘫患儿按随机数表法分为对照组与观察组。对照组采用鼠神经生长因子治疗,观察组采用靳三针联合鼠神经生长因子治疗。比较两组治疗前后运动功能、痉挛程度、血清白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平的变化情况,并对两组临床疗效进行评价。结果:观察组有效率为90.91%,显著优于对照组75.76%,差异有统计学意义(P<0.05);与治疗前比较,两组粗大运动功能评估量表(gross motor function measure,GMFM)、精细运动功能评定量表(peabody Development Motor Scales-2,PDMS-2)评分均上升,而改良Ashworth痉挛量表(modified ashworth scale,MAS)积分、血清IL-6、TNF-α水平均下降,且观察组均优于对照组,差异具有统计学意义(P<0.05);治疗期间两组均未出现不良反应及肝肾功能异常。结论:采用靳三针联合鼠神经生长因子治疗小儿痉挛型脑瘫可显著提高临床疗效,改善患儿运动功能,减轻痉挛程度,降低血清IL-6、TNF-α水平,促进患儿身体恢复。Objective： To observe the clinical efficacy of Jin＇ s Three-Needle combined with mouse nerve growth factor（ NGF） in treatment of children with spastic cerebral palsy（ SCP）. Methods： 132 SCP children who were admitted into Kaifeng traditional Chinese hospital from July 2015 to July 2016 were randomly divided into control group and observation group,with 66 cases in each group. The control group took mouse NGF,while the observation group took Jin＇s Three-Needle combined with mouse NGF.The changes of motor function,degree of spasm,serum interleukin-6（ IL-6）,tumor necrosis factor-α（ TNF-α） levels of two groups before and after treatment were compared,and the clinical efficacy of two groups were evaluated. Results： The total effective rate of observation group was 90. 91%（ 60/66）,which was significantly higher than that（ 75. 76%）,（ 50/66） of the control group（ P〈 0. 05）. Compared with that before treatment,the gross motor function measure（ GMFM）,Peabody Development Motor Scales-2（ PDMS-2） scores of two groups increased,but Modified Ashworth Scale（ MAS）,serum IL-6,TNF-α levels decreased.All of the above indexes of the observation group were significantly better than that of the control group（ P〈 0. 05）; There was no adverse reactions,liver and kidney dysfunction in two groups during treatment. Conclusion： Jin＇ s Three-Needle combined with mouse NGF in treating for SCP children can significantly increase the clinical symptoms,improve motor function,reduce degree of spasm,decrease serum IL-6 and TNF-α levels and promote patients＇ recovery,which has great clinical significance.
