苯扎氯铵对大耳白兔血管吻合手术模型动脉阻力指数、搏动指数的影响及意义

摘　　要：目的探讨苯扎氯铵对血管吻合手术模型大耳白兔动脉阻力指数(resistance index,RI)、搏动指数(pulse index,PI)的影响及意义,阐明不同浓度的苯扎氯铵溶液与血管壁刺激程度之间的关系。方法选取48只新西兰大耳白兔,随机分为对照组(生理盐水组)、苯扎氯铵溶液(Benzalkonium,BZK)0.01%、0.05%、0.1%组。制作血管吻合手术实验动物模型。检测分析各组实验动物术后左侧股动脉的阻力指数、搏动指数的变化。结果术后各组RI、PI均呈现增高后逐步减低的状态。术后6、12h对照组与BZK0.01%组比较差异无统计学意义(P>0.05),术后24h对照组与BZK0.01%组间差异具有统计学意义(P<0.05),术后6、12、24h BZK0.05%、0.1%组间比较差异无统计学意义(P>0.05),与对照组和BZK0.01%组比较差异有统计学意义(P<0.05)。结论低浓度0.01%BZK溶液对血管壁刺激性小于0.05%和0.1%浓度BZK溶液,有利于血管断端血流恢复,有效降低血管阻力,能够更好的舒张血管,缓解血管痉挛,对防止血栓形成有一定帮助,对吻合口通畅起到保护作用,提高血管吻合手术的成功率。Objective To study the effects and significance of benzalkoniumchloride on vascular anastomosis model of rabbit artery resistance index（resistance index,RI）,pulsatility index（pulse,index,PI）,and to elucidate the relationship between different concentration of benzalkonium chloride solution and the degree of stimulation of vascular wall.Methods Experimental animal group：48 New Zealand white big-ear-rabbits were divided randomly into control group（saline group）,benzalkonium（BZK）chloride solution 0.01%,0.05%,0.1% group.An experimental animal model of vascular anastomosis was established.The resistance index and pulsatility index of the left femoral artery were measured and analyzed in the experimental animals.Results After operation,RI and PI of each group were increased gradually and then gradually decreased.At 6 and 12 hours after operation,the difference between the control group and the BZK0.01% groups was not statistically significant（P〉0.05）,but at 24 hours after operation,there was significant difference between the control group and BZK0.01% group（P〈0.05）;after 6 hours,12 hours,24 hours,there was no significant difference between the 0.05% groups of BZK and 0.1%（P〉0.05）,compared with the control group and BZK0.01% group,the difference was statistically significant（P〈0.05）.Conclusion Low concentration of 0.01% BZK solution on the vascular irritation is less than0.05%and 0.1% concentration of BZK solution,in favor of the broken ends of vessels blood flow recovery,reduce vascular resistance,better blood vessels,alleviate vasospasm,to prevent thrombosis is helpful,and to protect the effect of anastomotic patency,improve the success rate of surgery for vascular anastomosis.
