摘 要:目的观察西宁及西安地区成人紫癜性肾炎血浆、尿液内皮素-1水平变化特征,分析其在判定西宁患者肾脏受损程度相关指数域。方法选择西安地区和西宁地区单纯性紫癜性肾炎患者各30例,健康人群各30例,共120例,分为西安地区肾脏损害组(18例)和无肾脏损害组(12例)、西宁地区肾脏损害组(19例)和无肾脏损害组(11例)作研究组,两地区健康人群组作健康对照组。采用ELISA方法测定血浆及尿液内皮素-1含量,并行组间差别分析。常规检测24小时尿蛋白定量,设定为亚组进行相关性分析。采用SPSS 22.0软件行统计学分析。结果 1.西宁地区紫癜性肾损害组、无肾损害组血浆、尿液内皮素-1与于西安地区相比较,结果具有统计学差异(P<0.05)。2.西宁地区紫癜性肾损害组的血浆内皮素-1水平与无肾损害组相比较,结果无明显统计学差异(P>0.05)。3.西宁地区紫癜性肾损害组的尿液内皮素-1水平高于无肾损害组,但结果无统计学意义(P>0.05)。进一步分析西宁地区紫癜性肾损害组中不同程度尿蛋白定量水平尿液内皮素-1的变化,结果显示:1.尿蛋白定量>1.0 g/24 h时尿液内皮素-1表达明显高于尿蛋白定量0.15 g^1.0 g/24 h和尿蛋白定量<0.15 g/24 h时,存在统计学意义(P<0.05);尿蛋白定量0.15 g^1.0 g/24 h组尿液内皮素-1水平高于尿蛋白定量<0.15 g/24 h组,存在统计学意义(P<0.05)。2.经Pearson相关性分析显示,紫癜性肾损害组患者尿液内皮素-1水平与24 h尿蛋白定量呈正相关(r=0.597,P<0.007)。结论高海拔地区紫癜性肾损害组患者血浆、尿液内皮素-1水平的变化具有地域性特点,要视具体情况判断不同程度紫癜性肾炎的肾脏受损情况。Objective In this study, we observed the change of Plasma and urine endothelin-1 (ET-1 )in adult patients with Henoch-schonlein purpura nephritis(HSPN) in Xining region, also, we analyzed the relativity of these changes with renal damage.Methods 30 cases with HSPN patients and 30 healthy cases in Xining and Xi'an region respectively, total 120 cases. Including 18 cases of Xining region with renal damage, the other patients were collected HSPN group in healthy people was used as control group in each region.The levels HSPN group in Xi'an region and 19 cases of in both regions are without renal damage ,while of ET-1 in plasma and urine were detected respectively by double antibody sandwich enzyme-linked immunosorbent assay( ABC-ELISA), and further, we ana- lyzed the difference in two group.At the same time,the levels of 24 h urine protein of each patients were routinely detected, and the correlation for ET-1 in urine and urine protein were analyzed. Finally, SPSS 22.0 software was used to analyze all data statistically.Results 1.ET-1 levels of Plasma and urine in both HSPN and HSP without re- nal damage group were significantly higher than that of the control group in the two regions(P〈0.05).2.The differ- ence without statistical significant(P〉0.05)in the level of plasma ET-1 of the HSPN group compared with the usp group in the Xining region.3.The difference without statistical significant(P〉0.05) in the level of urine ET-1 of the HSPN group compared with the HSP group in the Xining region.Further, we analyzed the changes of the quantitative level for urinary protein and the level of urinary ET- 1 in Xining region.The result showed : ( 1 ) The levels of urinary ET-1 were the highest in group Ⅲ (urinary protein〉 1.0g/24h), secondary were those in group Ⅱ (urinary protein 0.15 - 1.0g/24h)and then group Ⅰ (urinary protein 〈0.15g/24h), the difference between each group was statisti- cally significant( P〈0.05 ).
