腹膜透析患者透析充分性指标KT/Vurea与心功能的关系分析

摘　　要：目的探讨腹膜透析患者透析充分性指标尿素清除指数(KT/Vurea)与心功能相关指标的关系。方法选取2009年9月至2016年3月该院腹膜透析中心随访的持续不卧床腹膜透析(CAPD)患者72例,根据透析充分与否将患者分为透析充分组(42例,KT/Vurea≥1.7)和透析不充分组(30例,KT/Vurea<1.7),比较两组患者心功能指标脑钠肽前体(NT-pro-BNP)、左心室射血分数(LVEF)、心胸比例等差异,分析透析充分性与心功能相关指标的关系。结果两组患者心功能指标NT-pro-BNP、心胸比例、美国纽约心脏病协会(NYHA)心功能评分比较,差异均有统计学意义(P<0.05);而两组患者LVEF比较,差异无统计学意义(P>0.05)。透析充分性指标KT/Vurea与NT-pro-BNP、心胸比例、NYHA评分呈负相关(r=-0.390、-0.258、-0.554,P<0.05),与LVEF呈正相关(r=0.385,P<0.01)。结论透析充分性指标KT/Vurea与心功能评估相关指标关系密切,透析充分性可影响腹膜透析患者的心功能。Objective To investigate the relationship between the index of urea clearance index（KT/Vurea） and cardiac function in patients with peritoneal dialysis. Methods Totally 72 patients with continuous ambulatory peritoneal dialysis（CAPD）,who were followed up by Peritoneal Dialysis Center of Changshu No.2 People′ s Hospital,were selected from September 2009 to March 2016. According to the adequacy of dialysis,the patients were divided into dialysis adequacy group（n=42,KT/Vurea≥1.7）and inadequate dialysis group（n=30,KT/Vurea〈1.7）,and the difference of N-terminal pro-brain natriuretic peptide（NT-pro-BNP）,left ventricular ejection fraction（LVEF） level,cardiothoracic ratio and so on between the two groups were compared,in order to analyze the relationship between dialysis adequacy and correlative indicators of cardiac function. Results The difference of NT-pro-BNP,cardiothoracic ratio and cardiac function grading of NYHA between the two groups had statistical significance（P〈0.05）,but the difference of LVEF between the two groups showed no statistical significance（P〉0.05）. The KT/Vurea was negatively correlated with NT-pro-BNP,cardiothoracic ratio and NYHA scores（r=-0.390,-0.258,-0.554,P〈0.05）,while it was positively correlated with LVEF（r=0.385,P〈0.01）. Conclusion The KT/Vurea as an index of dialysis adequacy is closely related with the correlative indicators of cardiac function,and dialysis adequacy can affect the cardiac function of patients with peritoneal.
