摘　　要：家庭医生2型糖尿病健康素养管理方法的研究,旨在提高糖尿病患者疾病控制综合健康素养水平。以该研究设计的综合健康素养管理工具包为基础,通过家庭医生对签约服务的糖尿病患者实施精细化管理,为患者提供系统的糖尿病综合健康素养防治知识与技能;以半年为1个管理周期,通过分类管理、多种服务形式和管理效果测评,提升患者疾病自我管理能力,最终达到知行合一。The study of type 2 diabetes mellitus health literacy management by family doctors in community, aimed to improve pa- tients＇ diabetes health literacy of disease control. On the basis of integrated health literacy management package designed by the re- search, the family doctors implemented detailed management and provided comprehensive health literacy knowledge and skills for di- abetic patients signed contracts with the family doctors. A management cycle was half a year. Through the classification management, a variety of service form and effect were used assessment to enhance patients ability of disease self-management so as to make the unity of patients＇ knowledge and action.
