摘 要:目的调查广州天河区绝经后妇女维生素D水平及其与骨密度的关系。方法随机选取2015年4月至2015年8月在广州市天河区前进街社区卫生服务中心体检的109名绝经后妇女,年龄49~75岁。检测血清25(OH)D水平,分析血清25(OH)D水平与骨转换指标和骨密度(BMD)的相关性。结果 25.7%的受试者血清25(OH)D水平≥75 nmol/L,41.3%在50~75 nmol/L之间,33.0%低于50 nmol/L。双变量分析显示血清25(OH)D水平与血清PINP、β-CTX水平无相关性。股骨颈BMD与血清25(OH)D正相关(r=0.373,P=0.030),校正年龄和体重指数(BMI)后,相关性仍存在(r=0.174,P=0.049)。多元回归分析显示年龄、BMI、25(OH)D是股骨颈BMD的预测因子(R2=0.325)。结论绝经后女性广泛存在血清25(OH)D不足或缺乏,血清25(OH)D水平与股骨颈BMD正相关,适当补充维生素D对维持股骨颈区域的骨量至关重要。Objective To investigate the relationship between serum 25( OH) D level and bone mineral density( BMD) in Tianhe District of Guangzhou. Methods 109 postmenopausal women aged from 48 to 85 years old were randomly recruited from the Forward Street Community Health Center of Tianhe district of Guangzhou. BMD was measured at the lumbar spine and femoral neck. The level of serum 25( OH) D were assayed. The correlation between serum 25( OH) D level and BMD was investigated.Results The serum 25( OH) D levels were higher than 75 nmol/l in 25. 7% of the subjects,50-75 nmol/l in 41. 3% of the subjects,and lower than 50 nmol/l in 33. 0% of the subjects. Bivariate analysis showed that no significant correlations were found between serum 25( OH) D levels and serum PINP and β-CTX levels. Serum 25( OH) D levels were positively correlated with femoral neck BMD( r = 0. 373,P = 0. 030),and the correlation between serum 25( OH) D levels and femoral neck BMD persisted after adjustment of age and BMI( r = 0. 174,P = 0. 049). In multiple regression analyses,age,BMI,and serum 25( OH) D were predictors for BMD of the femoral neck( R2 = 0. 325). Conclusion The deficiency and insufficiency of serum 25( OH) D are serious problem in postmenopausal women. Serum 25( OH) D levels are positively related to femoral neck BMD. Thus,adequate supplement of vitamin D is crucial for the maintenance of bone mass in the femoral neck.
