NICU极低体质量新生儿非营养性吸吮和抚触的胃肠功能分析

摘　　要：目的探究非营养性吸吮和抚触对进入NICU病房极低体质量新生儿胃肠功能的影响分析。方法选择62例在本院NICU病房进行住院治疗的极低体质量新生儿作为研究对象,按进入NICU时间的先后分为观察组(33例)和对照组(29例),对照组新生儿仅给予单纯胃管喂养,观察组的新生儿则在胃管喂养间隙给予非营养性吸吮和抚触,对2组新生儿的体质量变化以及胃肠功能进行分析比较。结果出生时2组新生儿的体质量水平无差异(P<0.05),出生后7、14、28 d,观察组新生儿的体质量水平高于对照组(P<0.05),观察组在排便情况以及喂养相关情况上的各项指标均优于对照组(P<0.05)。此外,观察组新生儿在喂养期间出现的呕吐、腹胀、胃潴留和血糖异常的概率均低于对照组,而上述症状的改善时间也均低于对照组(P<0.05)。结论非营养性吸吮和抚触促进了极低体质量新生儿胃肠功能的发育成熟,使喂养不耐受的发生减少,从而加快了极低体质量新生儿早期的生长发育。Objective To explore the effect of non nutrition sucking and massage on gastro- intestinal function in very low birth weight infants in NICU. Methods A total of 62 very low birth weight infants in NICU of our hospital were selected as study subjects. According to the order of entering NICU time, they were divided into observation group （33 cases） and control group （29 cases）. The control group only treated with gastric tube feeding and the observation group was treated with non nutrition sucking and massage on the basis of the control group. The changes of body weight and gas- trointestinal function in two groups were analyzed and compared. Results There was no significant difference in birth weight of the neonates in two groups at birth （ P 〈 0.05）. At 7th, 14th and 28th day after birth, the body mass of the observation group were significantly higher than those of the control group （P 〈 0.05 ）. The indicators of defecation and feeding in the observation group were better than that of the control group （P 〈 0.05 ）. In addition, the incidence rates of vomiting, abdominal disten- tion, gastric retention and pathoglycemia of the observation group during feeding period were signifi- cantly lower, and the symptoms improved times were significantly lower than that in the control group （ P 〈 0.05 ）. Conclusion Non nutrition sucking and massage can promote maturation of gastrointestinal function, reduce the occurrence of feeding intolerance, accelerate the early growth and development of very low birth weight infants.
