血清SOD、CHE、D-D在丙型肝炎中的变化及临床意义

摘　　要：目的探讨血清超氧化物歧化酶(SOD)、胆碱酯酶(CHE)、D二聚体(D-D)在慢性丙型肝炎(HCV)中的变化及临床意义。方法选取2015年6月至2016年6月该院收治的148例HCV患者为研究对象,并根据患者病情分为急性肝炎组34例、慢性肝炎组36例、重症肝炎组40例及肝硬化组38例,另选取35例健康体检者为对照组,采用ELISA法测定各组血清SOD、CHE、D-D水平。结果随着HCV组患者病情的加重,患者血清总胆红素(TBil)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)及HCV-DNA载量水平显著升高(P<0.05)。与对照组相比,急性肝炎组血清SOD、CHE、D-D水平差异无统计学意义(P>0.05),而慢性肝炎组、重症肝炎组及肝硬化组血清血清SOD、CHE、D-D水平与对照组比较,差异有统计学意义(P<0.05)。且SOD、CHE随着患者病情加重而下降,而D-D随着患者病情加重而升高(P<0.05)。经Person相关分析,SOD、CHE与TBil、HCV-DNA呈负相关(P<0.05),D-D与TBil、HCV-DNA呈正相关(P<0.05)。结论检测HCV患者血清SOD、CHE、D-D水平对分析HCV病理机制及疾病发生、发展机制有重要的临床价值。Objective To investigate the changes and clinical significance of Serum superoxide dismutase（SOD）,cholinesterase（CHE）,D-dimer（D-D）in hepatitis C.Methods 148 cases of HCV patients were divided into acute hepatitis（n=34）,chronic hepatitis（n=36）,severe hepatitis（n=40）and cirrhosis（n=38）and 35 cases of normal subjects were selected as the control group from June 2015 to June 2016.The levels of SOD,CHE,DD of each groups were measured by ELISA.Results The levels of serum total bilirubin（TBil）,alanine aminotransferase（ALT）,aspartate aminotransferase（AST）and HCV-DNA of HCV group with the changes of the disease in patients（P0.05）.The levels of SOD,CHE,D-D of control group and acute hepatitis were not statistically significant（P0.05）,and the levels of SOD,CHE,D-D of chronic hepatitis,severe hepatitis and cirrhosis sera were higher（P0.05）.The levels of SOD,CHE of sicker patients were decreased,D-D with sicker patients was increased（P0.05）.The levels of SOD,CHE were negatively correlated with TBil,HCV-DNA（P0.05）,D-D was positively correlated with TBil,HCV-DNA（P0.05）.Conclusion Serum SOD,CHE,D-D level in HCV patients analysis of HCV disease mechanism,pathology and development mechanism have important clinical value.
