摘 要:目的探讨血清胆红素对冠心病患者血管内皮功能的影响并分析两者相关性。方法选择广东省农垦中心医院心血管内科2013年3月至2016年6月确诊为冠心病的112例患者为观察组,另选取同期参与门诊健康体检的112例健康人群为对照组;分别测定两组受检者的血清胆红素及内皮素-1(ET-1)、一氧化氮(NO)、血管内皮生长因子(VEGF)水平,并运用统计学方法分析血清胆红素与血管内皮功能的相关性。结果两组受检者的性别、吸烟史、年龄、高血压病史、体质量指数、高血脂病史及糖尿病史等一般情况比较差异均无统计学意义(P>0.05);观察组患者的总胆红素水平(TBIL)、间接胆红素(IBIL)、直接胆红素(DBIL)、NO及VEGF浓度均低于对照组,而ET-1浓度高于对照组,差异均具有统计学意义(P<0.05);观察组患者的血清胆红素与ET-1值均呈显著负相关(r=-0.376、-0.415、-0.442,P<0.05),而与VEGF值均呈显著正相关(r=0.471、0.368、0.425,P<0.05),间接胆红素与NO呈显著正相关(r=0.464,P<0.05)。结论血清胆红素可保护血管内皮功能,其在动脉粥样硬化发生、发展中发挥着重要作用,是冠心病发病的保护因素。Objective To study the influence of serum bilirubin level on vascular endothelial function of patients with coronary heart disease and to analyze the correlation. Methods A total of 112 patients with coronary heart disease, who admitted to Cardiovascular Department of Nongken Central Hospital of Guangdong Province from March2013 to June 2016, were selected as the observed group. Meantime, 112 cases of the healthy subjects of outpatients were chosen as the control group. The levels of serum bilirubin and endothelin 1(ET-1), nitrogen oxide(NO) and vascular endothelial growth factor(VEGF) of the two groups were measured, and the correlation between the level of serum bilirubin and vascular endothelial function was analyzed by statistical methods. Results There was no statistically significant difference between the observed group and the control group in the parameters of gender, age, history of smoking,body mass index, history of hypertension, history of diabetes and history of hyperlipidemia(P〈0.05); Compared with the control group, the levels of total bilirubin(TBIL), indirect bilirubin(IBIL), direct bilirubin(DBIL), NO and VEGF in the observed group were significantly decreased, and the levels of serum ET-1 was significantly increased(P〈0.05). The serum bilirubin level was negatively correlated with the level of ET-1(r=-0.376,-0.415,-0.442, P〈0.05) and positively correlated with the level of VEGF(r=0.471, 0.368, 0.425, P〈0.05) in the observed group, and the indirect bilirubin level was positively correlated with the level of NO(r=0.464, P〈0.05) in the observed group. Conclusion Serum bilirubin can protect the vascular endothelial function, and plays an important role in the occurrence and development of coronary heart disease, which is a protective factor of coronary heart disease.
