摘　　要：目的探讨自发性孤立性肠系膜上动脉夹层(SISMAD)患者治疗前后的CT血管成像(CTA)影像表现。方法回顾性分析2014年1月至2016年12月期间6例在海南省人民医院经CTA诊断为SISMAD(其中2例经DSA确诊,1例行支架置入,5例行保守治疗后复查)患者的临床资料,总结其治疗前后的CTA表现。结果 6例CTA扫描5例均见真假腔及破口,4例见假腔血栓形成,2例显示假腔出口,真腔小,假腔大。5例保守治疗后3例无明显变化,2例缩小。1例介入下溶栓加支架置入后,管腔复通。结论 CTA能够清晰显示SISMAD的破口、内膜片及血栓等,是确诊SISMAD及评价其治疗效果的首选影像检查方法。Objective To study the computed tomography angiography（CTA） imaging findings before and after the treatment of spontaneous isolated superior mesenteric artery dissection（SISMAD）. Methods The clinical data of 6 cases of SISMAD diagnosed by CTA in Hainan General Hospital from January 2014 to December 2016（2cases confirmed by DSA, 1 case treated with stent implantation, and 5 cases were reexamined after conservative treatment） were analyzed retrospectively. The CTA findings of SISMAD before and after the treatment were summarized. Results Among the 6 cases of CTA scanning, 5 cases showed true and false lumen and crevasse, 4 thrombosis occurred in 4 cases, and 2 cases showed false lumen exit. True lumen was small, and false lumen was big. Among the 5cases of conservative treatment, 3 cases had no significant change, and 2 cases had the reduction. One case were treated with stent implantation showed thrombolysis. Conclusion CTA scan can clearly show the dissection of the superior mesenteric artery dissection, intimal flap and thrombus, which is the preferred imaging method for the diagnosis of SISMAD and the evaluation of treatment effect.
