右心室间隔部起搏对心功能影响的临床研究

摘　　要：目的比较右室间隔(RVS)和右室心尖(RVA)起搏对心功能的影响。方法选择天长市人民医2014年7月至2015年11月收治的需要置入永久起搏器的患者54例,随机分A组(RVS起搏,26例)和B组(RVA起搏,28例),比较两组患者的起博参数、术后1个月、3月和6月心功能指标和NT-pro BNP差异。结果两组患者起博参数差异无统计学意义(P>0.05)。A组术后3个月和6个月的QRS时限和左室舒张末期内径低于B组,而左室射血分数高于B组,差异有统计学意义(P<0.05)。两组术后1周、1个月、3个月和6个月的NT-pro BNP差异无统计学意义(P>0.05)。结论与RVA起博相比,RVS起博能够改善患者的左室功能和重构。Objective To observe the effect of right ventricular septal（ RVS） pacing on cardiac function compared to right ventricular apical pacing in patients implanted with pacemakers. Methods Fifty-four patients were randomly divided into two groups： group A received RVS pacing（ n = 26） and group B received right ventricular apical（ RVA） pacing（ n = 28）. The pacing parameters,cardiac function indexes,and NT-pro BNP between the two groups were compared. Results There was no significant difference in pacing parameters between group A and group B（ P 〉 0. 05）. The mean QRS duration and LVEDD of group A were lower,and LVEF was higher than those of group B 3-month and 6-month after procedure（ P 〈 0. 05）. The plasma NT-pro BNP levels between the two groups were comparable 1-week,1-month,3-month and 6-month after pacemaker implantation（ P 〉 0. 05）. Conclusion Compared with RVA pacing,RVS pacing improves the left ventricular systolic function and remodeling.
