急诊哮喘患者静脉血气转化后与动脉血气的对比

摘  要:目的使用一种数学方法将急诊哮喘患者静脉血气转化为与动脉血气类似的结果 ,验证该方法的准确度。方法选择2015年4月—2016年4月该科就诊的40例哮喘急性发作患者,在其入院后的5 min内于同侧肢体采集静脉、动脉血标本,同时予以记录脉氧饱和度,对静脉、动脉血气以及转化后静脉血气的p H值、PCO_2、PO_2之间的关系进行观察研究。结果静脉血与动脉血对比,pH值、PCO_2、PO_2相关性好,r^2分别为0.95、0.83、0.74,偏倚与标准差[(0.04±0.06)、(-1.40±1.49)、(3.12±2.03)]皆高于实验室可接受误差范围;经转化后的静脉血与动脉血指标对比,p H值、PCO_2、PO_2相关性好,r^2分别为0.97、0.99、0.86,其中除了PO_2(1.35±1.58)高于实验室可接受误差范围外,另两项[(-0.01±0.04)、(0.04±0.40)]均在可接受误差范围内。结论经转化的静脉血气可提供与动脉血气相似的p H值、PCO_2结果,但两者PO_2结果相差较大,还需进一步的研究。Objective To transform the venous blood gas into a result similar to the arterial blood gas by using a method and verify the accuracy of the method. Methods 40 cases of patients with acute attack of asthma in our depart-ment from April 2015 to April 2016 were selected and the venous and arterial blood specimens were collected in 5 min after admission, at the same time, the correlation between the pH value, PCO2 and P〇 2 was observed and researched. Results The correlation between the pH value, PCO2 and P〇 2 of venous and arterial blood was good, r2 was respectively 0.95, 0.83, 0.74, and the bias and standard deviation were higher than those in the acceptable error range, which were [(0.04±0.06), (-1.40±1.49), (3.12±2.03)], after transformation, the correlation between the pH value, PC〇 2 and P〇 2 of venous and arterial blood was good, r2 was respectively 0.97, 0.99, 0.86, and PO2 was higher than that in the accept-able error range (1.35±1.58)kPa, the PC〇 2 and P〇 2 were in the acceptable error range, which were respectively (- 0.01±0.04) and (0.04±0.40). Conclusion The venous blood gas can provide the pH value and PCO2 similar to those of arterial blood gas, but the difference in PO2 result between the two is bigger, which needs the further research.
