摘  要:目的开展介入手术中术者眼晶状体和手指受照剂量调查,初步了解我省介入放射工作人员眼晶状体及手部剂量水平,并为给出相关的辐射防护要求提供一定的数据参考。方法 2014.7-2015.3选取两家综合大型医院进行现场调查,选择20名放射工作人员作为调查对象,共发放70套用于直接测量的指环及眼晶状体剂量计(剂量元件为Li F(Mg,Cu,P)),20套用于佩戴2个月后累积剂量调查;50套用于不同类型单台手术的剂量调查。结果 14位心内科介入诊疗人员中,眼晶体累积剂量最高达到2.64 mSv,手指剂量最高达到11.61 mSv;2位脑外科人员调查结果则分别为0.02、14.41 mSv;2位以TECA为主的介入手术人员结果分别为2.46、29.33 mSv。单次手术冠状动脉造影(CA)的眼睛体及手部剂量范围分别为0.11~0.27、0.11~0.70 mSv;冠状动脉腔内形成术(PTCA)分别为0.04~0.31、0.07~0.47 mSv;经皮冠状动脉介入治疗(PCI)分别为0.10~1.49 mSv、0.15~7.43 mSv;射频消融(RFA)分别为0.020~0.26 mSv、0.020~0.24 mSv。结论按此相当的工作量可粗估最高眼晶状体年剂量水平接近5年平均不超过20 mSv的IAEA国际限值标准,提示介入手术人员应加强眼晶状体监测及采取防护措施是必要的。Objective To investigate the operators'lens and finger radiation dose in interventional operation to get to know interventional radiology staff extremity radiation dose levels in Guangdong and to provide some reference data for giving relevant radiation protection requirements. Methods 2014.7 - 2015.3 20 interventional radiology staff in two comprehensive hospitals were selected as interviewees, who were issued 70 sets of rings and eye lens dosimeters (LiF (Mg, Cu, P) ). 20 sets of dosi- meters used to cumulative dose survey after wearing two months, while the others used to dose survey in different types single surgery. Results In 14 interventional cardiology staffs, the eye lens cumulative dose was up to 2.64 mSv, and hands dose was up to 11.61 mSv. 2 neurosurgery staffs survey results respectively while 2 TECA - based interventional surgical staffs were respectively 2.46 mSv, 29.33 mSv. The eye len and hand dose ranges in single surgical coronary angioraphy (CA) were re- spectively 0.11 - 0.27 mSv, 0. 11 - 0.70 mSv ; transluminal coronary angioplasty ( PTCA ) were respectively 0.04 - 0.31 mSv, 0.07 - 0.47 mSv ; percutaneous coronary intervention (PCI) were respectively 0.10 - 1.49 mSv, 0.15 - 7.43 mSv; ra- diofrequency ablation (RFA) were respectively MDL - 0.26 mSv, MDL - 0.24 mSv. Conclusion According to this consider- able workload, highest eye len annual dose levels were estimated roughly closer to international standard, suggesting that for surgical intervention personnel, to strengthen monitoring of the extremities and take protective measures are necessary.
