硝酸甘油软膏联合复方多粘菌素B软膏在腋臭术后的应用

摘　　要：目的探究硝酸甘油软膏联合复方多粘菌素B软膏对腋臭切除术区预后的影响。方法将30例腋臭患者随机分为实验组和对照组,每组各15例。实验组在术后即刻以及每次换药后于术区涂以硝酸甘油软膏与复方多粘菌素B混合制成的软膏,对照组则单独给予小油纱覆盖。并于换药时观察两组患者术区皮瓣颜色及有无皮瓣坏死,切口愈合情况;于每次换药后根据Wong-Baker量表评估记录患者的疼痛程度。结果⑴实验组患者均未发生皮瓣颜色发黑、发紫、溃疡、坏死及切口愈合不佳等情况;对照组2例出现皮瓣缺血性坏死,4例出现切口愈合不佳;两组比较差异具有统计学意义(P<0.05)。⑵实验组患者术后疼痛程度明显低于对照组(P<0.05)。结论在腋臭切除术后,即刻及每次换药后使用硝酸甘油软膏联合复方多粘菌素B软膏混合而成的软膏不仅可降低术区皮瓣发生溃疡或坏死等并发症的概率,同时也可降低患者术后的疼痛程度。Objective To explore the effect of nitroglycerin combined with compound polymyxin B ointment on the prognosis after axillary osmidrosis resection.Methods Thirty cases with axillary osmidrosis were randomly divided into an experimental gronp and control group,15 cases in each group.In the experimental group,the surgical region was coated with nitroglycerin combined with compound polymyxin B ointment immediately postoperative and after each change of dressing.The control group was covered with small oil gauze.The flap color,necrosis,and wound healing was observed at the time of dressing change,and the pain level was evaluated based on the Wong-Baker scale.Results （1） No flaps became black or purple,experienced ulcer,necrosis,or poor incision healing in the experimental group.In the control group,ischemic necrosis of the flap was found in 2 cases and poor incision healing was found in 4 cases.There was significant difference between the two groups （P〈0.05）.（2） The degree of pain in the experimental group was significantly lower than that in the control group （P〈0.05）.Conclusion After axillary osmidrosis excision,the application of nitroglycerin and polymyxin B ointment can significantly reduce the incidence of skin flap ulcer or necrosis and relieve pain postoperatively.
