复发性流产妇女内分泌影响因素分析

摘　　要：目的探讨复发性流产(RSA)的女性内分泌影响因素。方法选取119例RSA妇女作为观察组,另外选取正常孕前检查妇女175例作为对照组。检测基础性激素、甲状腺激素及抗体、空腹胰岛素(FINS)、空腹血糖(FPG)水平,比较观察组与对照组各种内分泌情况的差异。结果观察组血清促黄体生成素(LH)、睾酮(T)水平明显高于对照组,差异有统计学意义(t=5.408、2.376,均P<0.05);观察组甲状腺抗体阳性检出率明显高于对照组,差异有统计学意义(χ~2=8.276,P<0.05),抗甲状腺过氧化物酶抗体(TPOAb)水平明显高于对照组,差异有统计学意义(t=2.714,P<0.05);观察组FINS及胰岛素抵抗指数(HOMA-IR)水平明显高于对照组,差异有统计学意义(t=5.576、4.564,均P<0.05)。结论血清高LH、T,甲状腺抗体阳性以及胰岛素抵抗可能是导致RSA的内分泌因素。Objective To explore the endocrine influencing factors of recurrent spontaneous abortions （RSA）. Methods A total of 119 women with RSA were selected as observation group, and 175 women receiving normal pre-pregnaney examination were selected as control group. The levels of basic sex hormones, thyroid hormone and antibodies, fasting insulin （FINS） , and fasting plasma-glucose （FPG） were detected, and the differences of endocrine conditions between observation group and control group were compared. Results The levels of serum luteinizing hormone （LH） and testosterone （T） in observation group were statistically significantly higher than those in control group （ t-： 5. 408, 2. 376, both P〈0. 05 ）. The positive detection rate of thyroid antibody in observation group was statistically significantly higher than that in control group （X^2 =8. 276, P〈0. 05）. The level of anti-thyroid peroxidase antibody （TPOAb） in observation group was statistically significantly higher than that in control group （t=2. 714, P〈0. 05）. The levels of FINS and insulin resistance index （HOMA- IR） in observation group were statistically significantly higher than tlaose in control group （t = 5. 576, 4. 564, both P 〈 0. （）5）. Conclusion High serum levels of LH and T, positive thyroid antibodies and insulin resistance may be the endocrine factors that contribute to RSA.
