摘  要:目的探讨彩色多普勒血流成像(CDFI)联合超声声脉冲辐射力成像技术(ARFI)评价新生大鼠缺氧缺血性脑损伤(HIBD)的程度并观察病理组织学改变,为早期临床诊断和早期治疗新生儿HIBD提供依据。方法选择108只1周龄新生Wistar大鼠,随机分成HIBD组、手术组和对照组,每组36只。改良Rice法建立动物模型后,分别于术后3、12、24、48、72 h行彩色多普勒超声技术测定双侧大脑中动脉(MCA)的收缩期最大血流速度(Vs),舒张期末最大血流速度(Vd)及阻力指数(RI),行超声声脉冲辐射力成像技术测量脑组织的剪切波速度。测量结束后取脑组织HE染色观察。结果 HIBD组及手术组较对照组术后Vs明显降低,且HIBD组Vs低于手术组(P<0.05);术后3 h,HIBD组Vd与手术组及对照组比较有明显的增高(P<0.05);HIBD组及手术组RI较对照组术后有显著减低,HIBD组明显低于手术组(P<0.05)。术后各组的VTQ值均逐渐增高,且HIBD组及手术组与对照组比较增高明显(P<0.05),HIBD组较手术组增高亦明显(P<0.05)。病理结果示HIBD组右侧大脑皮层、海马等区域神经细胞减少,尤以海马区域减少更为明显;胶质细胞反应性增生,脑间质及血管周围水肿明显,室管膜区及脑室周围的脑实质内可见红细胞。结论彩色多普勒超声结合超声声脉冲辐射力成像可用于早期诊断并评价新生大鼠HIBD程度。Objective To evaluate the degree of hypoxic-ischemic brain damage (HIBD) in neonatal rats by color Doppler flow imaging (CDFI) combined with acoustic radiation force impulse imaging (ARFI), observe the histopathological changes, and provide a basis for early clinical diagnosis and treatment of HIBD in neonates. Methods A total of 108 neonatal Wistar rats aged one week were selected and randomly divided into HIBD group, operation group, and control group, 36 rats in each group. After establishment of animal model by Rice method, CDFI was used to detect peak systolic velocity (Vs) , peak diastolic velocity (Vd) , and resistance index (RI) of bilateral middle cerebral artery (MCA) at 3, 12, 24, 48, 72 hours after operation. ARFI was used to measure shear wave velocity of brain tissue. After the observation, the brain tissue samples were observed after HE staining. Results Vs values in HIBD group and operation group were sta- tistically significantly lower than that in control group, Vs value in HIBD group was statistically significantly lower than that in operation group ( P〈0. 05 ) . At three hours after operation, Vd value in HIBD group was statistically significantly higher than those in operation group and control group ( P〈0. 05 ) . RI values in HIBD group and operation group were statistically significantly lower than that in control group, RI value in HIBD group was statistically significantly lower than that in operation group (P〈0. 05 ) . After operation, VTQ values in the three groups increased gradually, and the increasing values in HIBD group and operation group were statistically significantly higher than that in control group ( P〈0. 05 ), the increasing value in HIBD group was statistically significantly higher than that in operation group ( P〈0. 05 ).
