摘  要:目的检测特发性矮小患儿血清IGF-1R、IGFALS蛋白的水平,探讨其在特发性矮小(ISS)患儿生长发育中的作用。方法收集就诊的特发性矮小儿童26例作为实验组(ISS组),抽取10例年龄、性别相匹配的正常身高儿童作为对照组。对他们进行全面的病史采集和体格检查,行生长激素激发试验,检测IGF-1和IGFBP3,并检测血清IGF-1R、IGFALS蛋白表达水平,比较两组中IGF-1R、IGFALS蛋白表达是否有差异,并分析IGF-1R、IGFALS蛋白表达与各项临床指标的相关性。结果(1)ISS组血清IGF-1R表达水平明显低于对照组,差异具有统计学意义(P<0.05),ISS组和正常对照组血清IGFALS蛋白水平差异无统计学意义(P>0.05)。(2)ISS组血清IGFALS蛋白及IGF-1R蛋白表达量均与身高差呈负相关(P<0.05),IGF-1R蛋白表达与年龄呈正相关(P<0.05),IGFALS蛋白表达与年龄无相关性(P>0.05)。结论 (1)IGF-1R和IGFALS蛋白表达水平越低可能使ISS患儿身高差更大,患儿越矮小。(2)IGF-1R缺乏可能是部分特发性矮小患儿的原因。Objective To investigate the levels of serum IGF-1R and IGFALS proteins in children with idiopathic short stature.Methods A total of 20 children with idiopathic short stature( ISS group) were enrolled,and 10 age-and sex-matched normal height children were selected as control group. The data,including a comprehensive medical history,physical examination,growth hormone stimulation test,were collected. The IGF-1 and IGFBP3 levels were detected,and the serum IGF-1R and IGFALS levels were measured and compared between the two groups. The correlation between the expressions of IGF-1R and IGFALS proteins and the clinical parameters was analyzed. Results(1) The serum IGF-1R protein expression levels in ISS group was significantly lower than that in control group( P〈0. 05),while the serum IGFALS protein level showed not statistically significant difference between ISS group and control group.(2)For the ISS patients,the expression of IGFALS protein and IGF-1R protein was negatively correlated with the difference in height( P〈0. 05),the expression of IGF-1R was positively related to age,but the expression of IGFALS protein was not correlated with age. Conclusion(1) For the ISS patients,the expression of IGF-1R and IGFALS proteins is related to the difference in height.(2)IGF-1R deficiency may be the cause of some idiopathic short stature in children.
