经皮背侧入路闭合复位双头加压空心螺钉固定腕舟骨骨折  

Fixed Scaphoid Fracture by Double-Headed Cannulated Compression Screws via Percutaneous Dorsal Approach and Closed Reduction

摘　　要：目的:探讨腕舟骨骨折经皮背侧入路闭合复位的临床疗效。方法:选取解放军第91医院2015年5月至2016年10月收治的19例腕舟骨骨折患者为研究对象,采用经皮背侧入路,双头加压空心螺钉固定,Herbert分型B1分型:10例,B2型:6例,B4型:3例。术中手法复位后,腕关节极度屈曲,触及舟骨近极斜坡处,在导针引导、C型臂透视下,沿舟骨轴线打入合适长度的双头加压空心螺钉。结果:所有患者获得随访,骨折均获得愈合,6~12周不等,平均愈合的时间为8周。所有患者无感染、骨折再移位、骨折不愈合、腕关节不稳定等并发症。结论:本方法符合微创理念,骨折愈合时间明显缩短。Objective To investigate the surgical techniques and outcome of percutaneous screws fixtion of scaphoid fracture via a dorsal approach. Method A total of 19 patients with scaphoid fractures were enrolled in this study from May 2015 to October 2016. The patients were treated with percutaneous dorsal approach and double head compression hollow screw. HerbertB 1： 10 cases, B2：6 cases, B4 type： 3 cases. Surgery after the reduction of the wrist, the extreme flexion of the wrist, hit the skull near the slope, in the guide guide, C-ann perspective, along the axis of the scissors into the appropriate length of the double-headed hollow screws. Results All patients were followed, both bone healing, bone healing time of 6-12 weeks, an average of 8 weeks. There was no infection, nonunion, traumatic arthritis and other complications. Gonclusion Percutaneous double-headed cannulated compression screw fixtion for the treatment of carpal scaphoid fracture is mini-invisive, shortening the healing time, improving the cure rate, the results were satisfactory.
