单开门椎管成形显微手术治疗颈椎管髓外硬膜内肿瘤的研究

摘　　要：目的探讨单开门椎管成形显微手术治疗颈椎管髓外硬膜内肿瘤围术期护理方法及疗效。方法对2005-01—2015-01在我院治疗的91例颈椎管髓外硬膜内肿瘤患者的临床资料进行回顾性分析,所有患者给予单开门椎管成形显微手术治疗并进行围术期护理。结果椎板开门节段(2.52±0.46)个,手术时间(97.42±16.95)min,失血量(81.38±21.07)mL;疾病类型:神经鞘瘤57例,脊膜瘤15例,肠源性囊肿4例,脂肪瘤15例。经1a随访治愈76例,改善15例,治疗前后颈椎曲度比较差异无统计学意义(P>0.05)。结论针对颈椎管髓外硬膜内肿瘤患者采用单开门椎管成形显微手术治疗并配合围术期护理取得良好效果,有效恢复脊椎原有结构,防止术后脊椎畸形,对脊柱影响小。Objective To investigate the perioperative nursing methods and curative effect in the single door laminoplasty microsurgery treatment for cervical intradural extramedullary tumor. Methods Totally 91 cases of cervical intradural extramedullary tumors in our hospital treatment from January 2005 to January 2015 were retrospectively analyzed. All patients were treated with unilateral open-door forming microsurgery and perioperative nursing. Results The opening section was 2.52±0.46, operation time was 97. 42 min±16.95 min, blood loss was 81.38 mL± 21.07 mL. Type of disease： 57 cases of neurilemmoma, 15 cases of meningioma,4 cases of enterogenous cysts,15 cases of lipoma. After 1 year follow-up, 76 cases were cured and 15 cases improved,patients showed similar cervical curvature before and after treatment （P〉0. 05）. Conclusion Unilateral laminoplasty combined with microsurgery treatment and perioperative nursing care can promote good therapeutic effect, effectively restore the original structure of the spine, prevent postoperative spinal deformity, and had little impact on the spine in ervical spinal intradural extramedullary tumors.
