糖尿病并发获得性肺部感染患者病原菌分布及药物敏感性研究

摘　　要：目的探讨糖尿病并发获得性肺部感染患者的病原菌分布及药物敏感性,为临床治疗合理选择抗菌药物提供依据。方法选取2013年1月-2016年12月医院收治的136例2型糖尿病合并医院获得性肺炎患者,采集患者痰液标本,进行菌株鉴定和药敏试验。结果 136例糖尿病合并医院获得性肺炎患者中,检出病原菌158株,其中革兰阴性菌99株占62.66%,以肺炎克雷伯杆菌和铜绿假单胞菌为主;革兰阳性菌46株占29.11%,以金黄色葡萄球菌和肺炎链球菌为主;真菌13株占8.23%;肺炎克雷伯菌和铜绿假单胞菌对亚胺培南、美罗培南、氨曲南、左氧氟沙星、阿米卡星、氯霉素耐药率均<10.00%,金黄色葡萄球菌和肺炎链球菌对利奈唑胺、万古霉素、替考拉宁耐药率均<10.00%。结论革兰阴性菌是糖尿病合并医院获得性肺炎的主要致病菌,临床治疗时,根据药敏试验结果合理选择抗菌药物治疗。OBJECTIVE To investigate the pathogen distribution and drug sensitivity of patients with diabetes mellitus complicated with acquired pulmonary infections,so as to provide the basis for rational selection of antibiotics in clinical treatment.METHODS A total of 136 patients with type 2 diabetes mellitus and hospital-acquired pneumonia admitted in our hospital from Jan.2013 to Dec.2016 were enrolled.Samples of sputum were collected for identification and susceptibility testing.RESULTS Among the 136 patients with diabetes mellitus and hospital-acquired pneumonia,158 strains of pathogens were detected.Among them,99 strains were gram-negative bacteria,accounting for 62.66% ,which were mainly Klebsiella pneumoniae andPseudomonas aeruginosa,46 strains were gram-positive bacteria,accounting for 29.11% ,which were mainly Staphylococcus aureus and Streptococcus pneumoniae,and 13 strain s were fungi,accounting for 8.23% .The drug resistant rates of K.pneumoniae and P.aeruginosato imipenem,meropenem,aztreonam,levofloxacin,amikacin and chloramphenicol were allunder10% ,and the drug resistant rates of S.aureus and S.pneumoniaeto linezolid,vancomycin and teicoplanin were all under 10% .CONCLUSION Gram-negative bacteria are the main pathogens of diabetes mellitus with hospital-acquired pneumonia.Antibiotics should be rationally chosen according to the susceptibility test results in clinical treatment.
