摘 要:目的研究应用荧光标记法评价及改进医院环境清洁质量,进而降低医疗保健相关感染(HAI)。方法应用荧光标记法对医院病区床单元及医疗办公设备高频接触物表进行干预前后清洁质量的评价。结果中度、高度和极高度危险区域高频接触物表(包括床单元及医疗办公设备)干预前共做荧光标记1681处,合格1096处,清洁合格率65.20%;干预后共做荧光标记1683处,合格1347处,清洁合格率80.04%,干预前后比较,差异有统计学意义(χ~2=93.106,P<0.001);床单元高频接触物表干预前后比较,床头桌、床头、床尾、摇把手干预前后差异有统计学意义(P<0.05),床栏、设备带干预前后差异无统计学意义,医疗办公设备高频接触物表清洁合格率干预前后比较,治疗室操作台面、综合治疗车、换药车/服药车、电脑键盘、护士站及医生办公室电话、监护仪和输液泵干预前后差异有统计学意义(P<0.05);换药室操作台面干预前后差异无统计学意义。结论应用荧光标记法能快速、直观评价及考核清洁人员是否按规范进行环境物表的清洁,及时发现医院感染风险,教育培训有针对性,是医院感染控制及环境清洁质量改进的有效管理工具。OBJECTIVE To apply the fluorescence marker in assessment and improvement of environmental cleaning quality of hospitals so as to reduce the incidence of healthcare-associated infections(HAI).METHODS The cleaning quality of bed units and surfaces of high-frequency contact objects was evaluated by using fluorescence marker before and after the intervention.RESULTS Totally 1681 fluorescence markers were made for the surfaces of highfrequency contact objects in medium,high and extremely high risk zones before the intervention,including the bed units and office equipments,of which 1096 were qualified,with the qualified rate of cleaning 65.20% .Totally1683 fluorescence markers were made after the intervention,of which 1347 were qualified,with the qualified rate of cleaning 80.04% ,and there was significant difference before and after the intervention(χ^2=93.106,P〈0.001).There was significant difference in the surfaces of high-frequency contact objects bed units before and after the intervention(P〈0.05);there was significant difference in the bedside table,bed head,bed tail,or shake handles before and after the intervention(P〈0.05),there was no significant difference in the bedrails or equipment belts before and after the intervention.There was significant difference in the qualified rate of cleaning of surfaces of high-frequency contact office equipments before and after the intervention(P〈0.05);there was significant difference in the operation tables of treatment rooms,comprehensive treatment vehicles,dressing change carts/drug carriers,computer keyboards,nursing stations,telephones in doctors′offices,monitors,or infusion pumps before and after the intervention(P〈0.05);there was no significant difference in the operation tables of dressing change rooms before and after the intervention.CONCLUSIONThe fluorescence marker may facilitate the rapid,intuitive evaluation and assessment of the cleanliness of the environmental objects surfaces,find out the risk factors for nosocomial infect
