摘　　要：目的分析IgA/C_3在IgA肾病中的辅助诊断价值。方法选取我院肾内科2010年3月~2016年12月收治的原发性肾小球疾病患者186例,其中IgA肾病患者98例(设为IgA组),非IgA肾病患者88例(设为非IgA组)。比较两组患者入院时血清IgA、C_3和IgA/C_3水平。比较IgA组入院时Ⅰ、Ⅱ级和Ⅲ、Ⅳ、Ⅴ级患者的血清IgA、C_3和IgA/C_3水平。采用受试者工作特征曲线(ROC曲线)分析血清IgA和IgA/C_3水平诊断IgA肾病的敏感性和特异性。结果 IgA组患者入院时血清IgA、IgA/C_3水平均显著高于非IgA组(P<0.05)。在IgA组患者中,Ⅰ、Ⅱ级患者血清IgA、IgA/C_3水平明显低于Ⅲ、Ⅳ、Ⅴ级患者(P<0.05)。血清IgA/C_3水平诊断IgA肾病的敏感性为94%,明显高于IgA水平(P<0.05)。结论血清IgA/C_3水平可反映IgA肾病病理分级,是辅助诊断IgA肾病的一种无创参考指标。Objective To analyze the value of IgA/C3 in the diagnosis of IgA nephropathy. Methods 186 patients with primary glomerulopathy in our department of Nephrology from March 2010 to December 2016 were selected, including 98 patients with IgA nephropathy （group IgA） and 88 non IgA nephropathy patients （non IgA group）. The serum levels of IgA, C3 and IgA/C3 were compared between the two groups. The serum levels of IgA, C3 and IgA/ C3 in patients with group IgA at grade Ⅰ, Ⅱ and Ⅲ, Ⅳ and Ⅴ on admission were compared. The receiver operating characteristic curve （ROC curve） was used to analyze the sensitivity and specificity of serum IgA and IgA/C3 levels in the diagnosis of IgA nephropathy. Results The serum levels of IgA and IgA/C3 on admission were significantly higher in the IgA group than that in the non IgA group （P 〈 0.05）. The levels of serum IgA, IgA/and C3 in patients with grade Ⅰ and Ⅱ were significantly lower than those of patients with grade Ⅲ, Ⅳ and Ⅴ in group IgA （P 〈 0.05）. The sensitivity of serum IgA/C3 level to the diagnosis of IgA nephropathy was 94%, which was higher than that of IgA （P 〈 0.05）. Conclusion Serum IgA/C3 level can reflect the pathological grading of IgA nephropathy, and is a noninvasive reference index for the assistant diagnosis of IgA nephropathy.
