拥抱抚触、非营养性吸吮联合干预对新生儿足底采血时心率和疼痛的影响

摘　　要：目的观察抚触、非营养性吸吮联合干预对新生儿足底采血时心率和疼痛的影响。方法选择在我院住院的90例需足跟采血的新生儿作为研究对象,按随机数字表分为拥抱抚触组(Ⅰ组)、非营养性吸吮组(Ⅱ组)、联合组(Ⅲ组),每组各30例,分别采用拥抱抚触、非营养性吸吮及联合护理干预,比较三组新生儿穿刺前、穿刺后1min、穿刺后3min、穿刺后5min心率及疼痛评分(NFCS评分、NIPS评分),记录哭声持续时间、出血时间和出血滴血直径。结果Ⅰ组、Ⅱ组、Ⅲ组穿刺后1min、穿刺后3min、穿刺后5min心率差异有统计学意义(P<0.05),Ⅲ组穿刺后1min、穿刺后3min、穿刺后5min心率明显低于Ⅰ组、Ⅱ组(P<0.05)。Ⅰ组、Ⅱ组、Ⅲ组NFCS评分、NIPS评分差异有统计学意义(P<0.05),Ⅲ组NFCS评分、NIPS评分均明显低于Ⅰ组、Ⅱ组(P<0.05)。Ⅲ组哭声持续时间、出血时间明显短于Ⅰ组、Ⅱ组(P<0.05),出血滴血直径明显大于Ⅰ组、Ⅱ组(P<0.05)。结论拥抱抚触、非营养性吸吮联合干预能有效缓解新生儿疼痛,提高采血效果,且效果优于单一拥抱抚触或非营养性吸吮,值得临床推广使用。Objective To observe the influence of caresses touches and non-nutritive sucking on the heart rate and the pain degree. Methods 90 newborns needed to heel blood were selected as the subjects, they were divided into caresses touches group（group Ⅰ ）,non-nutritive sucking group（group Ⅱ） and Joint Group （Group Ⅲ）,30 cases of each group.They were respectively received caresses touches,non-nutritive sucking and joint nursing intervention. The Heart rate and pain scores（NFCS score,NIPS score）at lmin,3min,5min before and after the heel prick were compared,the duration of crying,bleeding time and bleeding were recorded. Results There was a difference in the Heart rate in Ⅰ group, Ⅱ group, Ⅲ group at 1min,3min,5min after the heel prick（P 〈 0.05）,the Heart rate of group Ⅲ were lower than group Ⅰ ,group Ⅱ at 1min,3min,5min after the heel prick（P 〈 0.05）.There was a difference in the scores of NFCS and NIPS in group Ⅰ ,group Ⅱ ,group Ⅲ （P 〈 0.05）,the scores of NFCS and NIPS in group Ⅲ were significantly lower than group Ⅰ ,group Ⅱ （P 〈 0.05）.The crying time,bleeding time of group Ⅲ were significantly shorter than those ingroup Ⅰ ,group Ⅱ （P 〈 0.05）,the Bleeding Diameter of Blood were significantly higher than group Ⅰ ,group Ⅱ （P 〈 0.05）. Conclusion The combination of caresses touches and non-nutritive sucking can effectively relieve neonatal pain,and improve the blood collection effect.The effect is better than caresses touches or non-nutritive sucking,which is worthy of clinical use.
