磷酸肌酸钠联合曲美他嗪治疗急性充血性心力衰竭的疗效观察

摘　　要：目的观察联用磷酸肌酸钠和曲美他嗪治疗急性充血性心力衰竭的疗效。方法选择120例急性充血性心力衰竭患者,随机分为观察组和对照组,每组60例。对照组仅应用洋地黄、利尿剂、扩张血管药物等常规药物治疗。观察组在常规抗心衰治疗的基础上加用磷酸肌酸钠和曲美他嗪。结果治疗后观察组的心功能指标如射血分数(EF)明显升高,心胸比(CTR)明显降低,改善程度明显好于对照组,差异有统计学意义(P<0.05);治疗后两组舒张末期左室内径(Dd)、舒张期室间隔厚度(IVST)、舒张期左心室后壁厚度(PWT)、左心室质量(LVMW)均有所改善,观察组的改善效果明显好于对照组,差异有统计学意义(P<0.05);观察组心脏血清学指标如脑钠肽(BNP)、N端前脑钠肽(NT-pro BNP)和氧化应激的指标超氧化物歧化酶(SOD)、丙二醛(MAD)的改善情况优于对照组。观察组的治疗显效率为76.67%,高于对照组的68.33%(P<0.05)。结论在常规药物治疗的基础上,联用磷酸肌酸钠和曲美他嗪治疗急性心功能不全可以取得更好的治疗效果。Objective To explore the curative effect of creatine phosphate sodium combined with trimetazidine in the treatment of acute congestive heart failure. Methods Totally 120 patients with acute congestive heart failure were randomly divided into treatment group and control group,60 cases in each group. Control group was only treated with rountine drugs,such as digitalis,diuretics and vasodilator drugs,while treatment group was treated with creatine phosphate sodium and trimetazidine besides the conventioal therapy of coutrol group. Results After treatment,the cardiac function indexes,such as ejection fraction（ EF）,in observation group increased significantly,the cardiothoracic ratio（ CTR） decreased significantly,and the improvement of them was better than that of control group（ P〈0. 05）. After treatment,the left ventricular end diastolic diameter（ Dd）,interventricular septal thickness（ IVST）,left ventricular diastolic posterior wall thickness（ PWT） and left ventricular mass（ LVMW） of the two groups were improved,and the improvement of observation group was better than control group（ P〈0. 05）. The indexes of cardiac serology in observation group,such as brain natriuretic peptide（ BNP） and N terminal forebrain natriuretic peptide（ NT-pro BNP）,and oxidative stress indexes,such as superoxide dismutase（ SOD） and malondialdehyde（ MAD） were improved significantly than control group. The effective rate of observation group was higher than that of control group（ 76. 67% vs.68. 33%,P〈0. 05）. Conclusion On the basis of conventional drug treatment,combination use of phosphocreatine sodium and trimetazidine for acute congestive heart failure can achieve better therapeutic effects
