肝素钠封管液预防静脉留置针相关并发症效果的Meta分析

摘　　要：[目的]了解肝素钠封管液减少静脉留置针相关并发症的效果。[方法]计算机检索PubMed、Elsevier、CALIS、中国知网、万方数据库、维普中文科技期刊数据库、Cochrane临床对照试验资料库、中国生物医学文献数据库,查找肝素钠封管液对静脉留置针相关并发症影响的随机对照试验。根据制定的纳入标准与排除标准,独立进行文献筛选及质量评价,采用Cochrane协作网RevMan 5.3软件进行Meta分析。[结果]最终纳入11个随机对照试验,共1 383例静脉留置针病人。Meta分析结果显示,与生理盐水封管相比,给予肝素钠封管液可以减少静脉留置针堵管的发生[OR=0.28,95%CI(0.19,0.41),P<0.000 01],减少血栓的发生[OR=0.12,95%CI(0.04,0.35),P<0.0001],延长静脉留置针留置时间[SMD=13.09,95%CI(10.62,15.55),P<0.000 01]。[结论]对静脉留置针病人给予肝素钠封管液可以有效预防静脉留置针堵管的发生及血栓的形成,并延长静脉留置针留置时间,但由于本次纳入的文献有限,且临床上RCT研究较少,有待高质量的RCT加以验证。Objective：To know about the effect of heparin sodium sealing liquid for reducing venous indwelling needle related complications. Methods：Computer retrieval was carried out in PubMed, Elsevier, CALIS, CNKI, Wanfang database, VIP, Cochrane controlled clinical trial database, Chinese biomedical literature database to search the randomized controlled trial on the influence of heparin sodium sealing liquid on venous indwelling needle related complications. According to the established inclusion criteria and exclusion criteria, independent screening and quality evaluation on literatures was carried out by Meta analysis with RevMan 5.3 software in Cochrane collaboration network. Results： Eventually a total of 11 randomized controlled trials were included, 1 383 cases of patients with venous indwelling needle. Meta-analysis results showed that, compared with control group,giving heparin sodium sealing fluid could reduce the occurrence of intra - venous indwelling needle tube jam[OR=0.28,95 %CI（0. 19,0.41）, P〈0. 000 01] ;reduce the occurrence of thrombosis[OR= 0.12,95% CI（0. 04, 0.35） ,P〈0. 000 01]; and prolong indwelling time of intravenous indwelling needle[SMD= 13. 09,95% CI（10.62,15.55） ,P〈0. 000 01]. Conclusion. Heparin sodium sealing fluid in patients with venous indwelling needle could effectively prevent the occurrence of intra venous indwelling needle tuke jam and the formation of blood clots,and to extend the time of indwelling time of intra - venous indwelling needle. Because literatures were limited, and RCT clinical studies were less,it remained to be verified by high-quality RCT.
