“互联网+”在社区糖尿病健康管理中的效果评价

摘　　要：目的:探索在社区服务中,利用"互联网+"方式,开展社区管理模式远程教育,并评价这种社区糖尿病管理模式的作用。方法:在广西中医药大学附属瑞康医院南城分院社区选取在册管理2型糖尿病患者80例,并将其随机分为2组,实验组和对照组各40例。实验组采用强化式健康管理模式,开展糖尿病远程教育,利用计算机视频,微信互动平台提供网络化的糖尿病知识,定期随访管理;对照组采用一般健康管理模式,仅发放宣传资料由患者学习。结果:干预18个月后,实验组糖尿病患者的相关知识知晓率有明显提高(P<0.05),采取措施控制血糖明显增多(P<0.05),对照组无明显变化。结论:这种新型的"互联网+"的社区糖尿病管理模式,可有效的提高糖尿病患者的自我管理能力,这种基于"互联网+"的社区糖尿病健康管理模式将会得到更好的推广。Objective：To explore the role of community management model in community service,using ＂Internet plus＂,to carry out the community management mode remote education,and to evaluate the role of the community diabetes management model.Methods：80 patients with type 2 diabetes were selected and randomly divided into 2 groups and 40 patients in the experimental group and the control group.The experimental group adopted aggrandizement health management mode,carried out diabetes remote education,used computer video,We Chat interactive platform to provide networked diabetes knowledge,and regular follow-up management;the control group adopts general health management mode and only disseminate information from patients.Results ： 18 months after the intervention,the experimental group with diabetes related knowledge awareness has increased significantly（P0.05）,and take measures to control blood sugar significantly increased（P0.05）,the control group no significant change.Conclusion：This new type of ＂Internet ＋＂ community diabetes management mode,which can effectively improve diabetes self-management ability,this kind of community based on ＂Internet ＋＂ diabetes health management mode to get better promotion.
